What are the responsibilities and job description for the PRN Hospice Social Worker (LMSW) position at Three Oaks Hospice?
Licensed, Master's Level Social Worker: PRN
Ideal candidate will be available 1-2 days per week, plus occasional on-call which could include weeknights and weekends as needed.
- Provide psychosocial on call coverage within a monthly schedule. Call coverage is usually in one week intervals from Sunday to Saturday; weeknights and weekend day/night) shared coverage with FT SW and our Chaplain.

We are looking for a passionate Licensed Master's Level Social Worker to join our leading team of hospice care professionals and make a difference, by listening, caring, and serving.
POSITION SUMMARY: Primary function is to provide coordinated care in the homes to patients of all age groups and perform psychosocial assessments, analysis, counseling, and referrals to meet the needs of the patient/family.
QUALIFICATIONS:
- Master’s degree in social work from a State accredited program.
- One (1) year social worker experience in a health care agency.
- Working knowledge of community resources. Good interpersonal skills. Hepatitis profile.
- Current State License: Texas Licensed MSW
- Reliable transportation. Valid Driver’s License and current auto liability insurance
