Responsible to: Director of Community Services
Job Summary:
The Family Specialist will provide direct service and case management to families in their homes. The Family Specialist will provide referral services to families, assisting the family in achieving the goals identified in the In-Home Service Plan. The Family Specialist completes the initial family contact and will direct the frequency and intensity of subsequent in-home services for assigned families.

Primary Functions:
- Provides family, individual, and group counseling to assigned families.
- Models child-rearing practices, behavior management techniques, and anger management.
- Conducts initial family assessment and directs the development of the In-Home Service Plan.
- Plans and coordinates program services and activities provided to the family by team members.
- Responds to the initial referral and subsequent crisis for assigned families.
- Responsible for a minimum of 33 billable hours each week.
- Responsible for case management of 3-4 designated families and specialist responsibilities for 4-5 designated families.
- Maintains monthly in-home service plans for designated families.
- Maintains professionalism at all times when working with families, Family Partnership staff, and outside agencies.
Other Responsibilities:
- Attend court hearings as required
- Participate in the review and revision of In-Home Service Plans.
- Attend internal and external training as required.
- Attend program staff meetings and meet with supervisory staff and other agency personnel as assigned.
- Participate in monthly file reviews
- Responsible for on-call duties during designated rotation, and response to the family crisis as necessary.
- Responsible for carrying an agency cell phone at all times and returning calls within fifteen minutes.
- Responsible for the weekly submission of paperwork.
Working Conditions:
While performing the duties of this job, the employee is regularly required to talk and hear. The employee frequently is required to stand; sit, and walk; use hands; reach with hands and arms; the employee must occasionally lift or move up to 25 pounds. Specific vision abilities required by this job include close vision and the ability to adjust focus. Proper diction and command of the English language is required. Must be able to exercise independent thinking and good judgment under all circumstances.
Qualifications:
A Bachelors degree in a human services area and a minimum of one-year in-home work with youth and families is preferred but not required. Excellent written and oral communication skills. Cultural competence is required. Act 33/34 clearances, a valid Pennsylvania driver's license and reliable transportation.
