Position Summary
The Clinical Care Coordinator I primary responsibilities are focused on managing high risk patients by assisting their navigation through the healthcare system, preventing unnecessary ER use, and preventing the need for hospitalizations and re-admissions and encouraging generic medication use.
Major Duties and Responsibilities
Required to visit patients in the hospital and at home to gather additional information and provide patient/caregiver teaching and education, as well as coordinate care with external organizations and provide resources as needed.
Will assess patient compliance, home care needs, DME needs, as well as other needs and issues.
Work closely with insurance companies to meet shared savings goals.
Participate in clinical trainings and assist in coordination or provision of continuing education for clinical staff.
Serve as a resource to all staff about patient care coordination, availability, and any additional pertinent information.
Participate in ongoing NextGen training and assist the clinical team in optimized development and use of the EMR system.
Participate in quality improvement initiatives and attend team meetings as directed.
Other duties as assigned.
Knowledge, Skills and Abilities
Knowledge of appropriate medical terminology.
Knowledge of computer systems and applications such as Excel, Word.
Skill in communicating clearly with diverse publics.
Skill in planning, organizing, delegating and executing tasks.
Skill in providing excellent customer service.
Skill in writing Plans of Care.
Ability to work scheduled hours as defined in the job offer.
Ability to establish/maintain effective working relationships with patients, medical staff and the public.
Ability to read and understand oral and written instructions and follow written protocols.
Ability to document and maintain records and test results.

Qualifications
Education / Training / Requirements
High School Diploma/GED.
Active licensure as a Registered or Licensed Nurse in Virginia.
Minimum of 1 years experience with direct patient care.
Physical Demands
Ability to stand and walk for long periods of time.
Ability to sit for extended periods of time.
Ability to climb or balance for limited periods of time.
Ability to occasionally reach, bend, stoop and lift up to 50 lbs. *
Ability to grasp and hold up to 25 lbs.*
