What are the responsibilities and job description for the Director-Case Management position at Timberline Knolls?
- Responsible for planning, staffing and budgets for the care management function.
- Manage resources and expenses, recommend and implement changes to the processes/methods utilized by the care management team.
- Execute the model and framework for the facilities continuing care services.
- Facilitate the operation of the continuing care program in collaboration with the leadership of the facility by using evidenced-based guidelines and medical criteria.
- Review initial clinical information format and presentation and evaluates the continuing application of these guidelines.
- Facilitate active patient and family participation in the treatment process.
- Maintain documentation in a timely, thorough and accurate manner.
- Hire, supervise and evaluate continuing care staff.
- Establish policies and procedures for continuing care program.
- Collaborate with facility leadership in the implementation of outcome studies for the continuing care program.
- Responsible for community outreach and marketing activities for the facility as required.
- Participate in quality improvement, program services review and consistently reviews client programming for best outcomes.
Education
Required- Bachelors or better
- Masters or better in Psychology
Licenses & Certifications
Preferred- Lic Social Worker
- Lic Clin Prof Cnslr
