To assess each resident’s condition at admission, quarterly, annually, and when significant and permanent changes in resident’s condition occur. To assist in the development of a comprehensive care plan to address the needs of each resident.
Responsibilities
- Assist with evaluation of residents prior to admission for appropriate placement.
- Schedule admission conference with resident and family to address any resident/family concerns and to review facility policies.
- Review all required admission documentation prior to admission to assure compliance with regulations.
- Review admission orders to assure compliance regulations.
- Perform comprehensive assessment within 14 days of admission, and annually, reviews assessment quarterly, and when significant and permanent changes occur in resident’s condition.
- Monitor resident’s condition during first 14 days of admission and assure accurate and complete documentation is available in medical record.
- Develop an interim plan of care to meet the individual needs of the resident.
- Monitor resident’s condition to assure documentation reflects resident’s condition.
- Schedule specific time for resident and family to attend Interdisciplinary Care Planning Conference.
- Schedule and coordinate Interdisciplinary Care Panning Conference.
- Identify resident’s problems, potential problems and areas of high risk prior to care planning conference.
- Recruit, select and train personnel to provide proper and continuous care for all residents.
- Assign duties to professional nurses and non-professional personnel. Supervise and evaluate work performance.
- Administer medications and treatments on assigned unit as directed by the Director of Nursing Services.
- Assume responsibilities of on-call RN as directed by the Director of Nursing Services.
- Develop care plan to meet the current needs of the resident and with input from resident and family and in conjunction with the Interdisciplinary Care Planning Committee.
- Assist in the evacuation of residents from the facility in a disaster.
- Provide emergency nursing measures in the event of a medical crisis.
- Must be able to position, transfer, ambulate, and assist with lifting residents during routine daily care and/or emergencies.
- Assign and directs duties of Nursing Assistant staff designated to provide care in specific area.
- Administer appropriate oral and written disciplinary actions to Nursing Assistant staff in assigned areas for violation of facility policies and procedures. Makes recommendations concerning suspensions and discharges.
- Review and evaluate, in writing Nursing Assistant staff in assigned areas.
- Support this facility’s commitment in the quality of services to residents, family members, and the public.
- Be able to hear, speak, and see.
- Be able to be on feet for several hours while bending, stooping, stretching, and lifting to assist residents.
- Be able to operate a Hoyer Lift, whirlpool, wheelchairs, and gait belt.
- Assist in the development and implementation of nursing Service’s annual departmental and capital budgets.
- Hold confidential all company, facility, resident, or employee information.
- Report all incidents/accidents immediately to supervisor.
- Attendance at all required in-service training programs and staff meetings.
- Adhere to facility established policies and procedures.
- Perform other duties as assigned or required.
Qualifications
Must be a Registered Nurse and currently licensed as a Registered Nurse in the state in which the facility is located. In performing the essential functions of this job, with or without reasonable accommodations, the individual cannot pose a direct threat to property, themselves, or others. Regular attendance must be maintained by the individual holding this position.
