TCS’ Coordination of Community Services program supports people of all ages with intellectual and developmental disabilities and their families. Our services are designed to help them learn about and gain access to resources, services, and supports in their community. Our goal is to empower them to make informed choices about their care and plan for their future.
Our Coordination of Community Services program helps our clients plan for and receive support services in their community through one of three 1915(c) Medicaid home and community-based waiver programs including the Family Supports, Community Supports, and Community Pathways Waivers.
What You Do:
- Identify client needs, goals, and preferences - what's important to them and for them.
- Inform them about important community resources and support that are available.
- Empower them to make informed choices so they can live a full life in the community.
- Help them gain access to the DDA and other service delivery systems, services, and resources.
- Monitor their services to make sure they continue to meet their needs and wants.
Salary : $60,000
