QUALIFICATIONS
- LVN or RN with Knowledge of healthcare reimbursement, case management and discharge planning is required.
- Understanding SNF regulatory requirement

Team Touchstone Directors of Care Solutions/Case Managers are responsible for the following:
- Exercise clinical expertise, knowledge and decision making to ensure successful discharge planning and care management
- Ensures necessary information is communicated to IDT, direct care staff, patient and responsible party/caregiver
- Facilitates the 72-hour and Care Connect meetings
- Responsible for the ongoing communication to the case management team
- Coordinates Care reviews
- Guarantees patients are monitored for change of condition
- Collaborate with the interdisciplinary team to develop individualized plans of care
- Leads successful transition of care though continued recovery once at home
- Provides education and support to patients and families about care processes, discharge expectations and resources
- Serve as member of the QAPI committee
- Director of Care Solutions is aware of the Balanced Scorecard and how their position contributes to success in Balanced Scorecard measures
