What are the responsibilities and job description for the Occupational Therapist - Home Health position at Touchstone Health?
Touchstone Health, a full-service Home Health and Hospice agency, is seeking a qualified and compassionate Occupational Therapist to join our Team. Under the supervision of the Administrator and in accordance with the interdisciplinary plan of care, our therapists evaluate, establish, and implement individual programs of care for our patients to increase functional ability and reestablish independence at home.
Duties & Responsibilities:
- Performing home visits to evaluate patient's level of function
- Developing an individual plan of care based on the client's personal goals
- Implementing the treatment program through direct treatment of the patient
- Reassessing clinical signs and symptoms to determine effectiveness of program
- Instructing patient, family, and staff regarding participation in the treatment program
- Collaborating with PT and additional interdisciplinary staff member to improve treatment program
Qualifications & Requirements:
- Degree in Occupational therapy from a university accredited by the Council of Medical Education of the Medical Association and the American Occupational Therapy Association
- License to Practice by the Texas
- Comprehensive knowledge of the principles, concepts, and methodology required of an occupation therapist and its practice application in the treatment of home health patients at home
- Excellent Communication and documentation skills
- Willingness to work with a interdisciplinary team
- Valid Texas driver’s license and reliable, insured transportation
- Travel San Antonio and surrounding area
