Housing Case Manager Float (Hybrid) - Minneapolis / Hennepin County
Pay Range: $47,000 to $50,000 annual salary, based on qualifications
Benefits: Full time, benefit eligible
Schedule: 5 Day Workweek (8 hours per day) or 4 Day Workweek (10 hours per day)Location: Hybrid (Clients are in Minneapolis/ Hennepin County)
The Housing Case Manager provides housing stabilization services with tenancy support and education, transition and sustaining services. The Housing Case Manager will also provide coverage for staff on Medical Leave in Case Management and Care Coordination.
Housing Case Manager Duties:
- Regularly meet with clients in their homes and other locations throughout the community.
- Establish supportive and motivational relationships with clients.
- Review and monitor services provided to clients.
- Connect clients to community resources and programs.
- Maintain client records in electronic health record system.
- Collaborate with other family members, service providers, staff members, consultants, interns and/or volunteers.
Housing Case Manager Requirements:
- Experience working with individuals who have experienced homelessness is highly desired
- Experience working with individuals affected by mental health and/or chemical dependency issues highly desired
- Bachelor's degree in a behavioral health field preferred
- Valid driver's license, vehicle insurance, and acceptable driving record required
