Position Type:    Part Time (20 hours or more)
Salary Range:    $33.48 - $41.85 Hourly

POSTED JULY 3, 2024 AND WILL REMAIN OPEN UNTIL FILLED.

THIS PART-TIME ROLE IS 24 HOURS PER WEEK WITH A VARIED SCHEDULE. NOT ELIGIBLE FOR FT BENEFITS, BUT  PAID TIME OFF INCLUDING HOLIDAY, PERSONAL, VACATION AND SICK IS PROVIDED.

 

GENERAL DESCRIPTION:

Under broad supervision, incumbent provides outreach services encompassing direct client assistance, crisis intervention, case management, public education, support groups, and resource coordination for seniors, disabled adults and financially at-risk individuals and families.

ESSENTIAL DUTIES:  

  1. Provides comprehensive outreach services, encompassing direct client assistance, on-site services, and home visits.   With client, develops action plans to address problems, monitor progress towards goals, reassess needs, and adjusts action plans.
  2. Acts in response to crisis situations and case management referrals to assist clients with the resolution of difficult issues, including but not limited to eviction/ejectment.  Assesses needs and applies counseling techniques to address clients’ presenting concerns in crisis. 
  3. Serves as client advocate and collaborates with other providers to link clients to appropriate services and other community support systems, programs and services. Evaluates eligibility for benefits such as the Energy Assistance, Medicare, Medicaid, Supplemental Nutrition Assistance Program, Uniform Relocation Assistance Act, and other federal, state, and local programs.
  4. Coordinates community resources and volunteers to provide services for financially disadvantaged individuals in the community such as the local food and fuel banks, and other department programs. Works with community groups and civic organizations to coordinate programs for seniors, disabled adults and financially at-risk residents in the community.  
  5. Coordinates support groups and educational series to benefit clients, their families and caregivers and the community.
  6. Maintains records of Outreach Services activities, case notes and files using an electronic database system.
  7. Keeps abreast of changing legislation, that affects seniors, disabled adults and individuals and families in financial need, and services and programs available to meet those needs.
  8. Markets programs and events, and shares information with the community, using desk top publishing, press releases, public speaking engagements and social media.
  9. Contributes to a respectful and safe work environment by fostering a culture of positive communication and ethical behavior, with an emphasis on providing exceptional service to internal and external customers.

DEPENDABILITY:

  • Regularly attends and is punctual for work
  • Occasionally, may be requested by public safety personnel to respond to case management crises, during evening or weekend hours

Qualifications


QUALIFICATIONS PROFILE:

  • Knowledge of
    • the issues and needs of the senior, disabled adult and the financially at-risk population and the resources available to meet those needs.
    • state traffic laws
  • Ability to
    • communicate effectively, orally and in writing, and establish and maintain effective working relationships with community agencies, staff and residents
    • give full attention to what others are saying and ask appropriate questions

    • use judgment in interpreting and adapting guidelines such as organizational policies, regulations, precedents and directions for application to specific cases
  • Skilled in
    • social work practice including engagement, assessment, intervention, monitoring and evaluation 
    • crisis intervention
    • presenting in public
    • Microsoft Office suite, including Word and Excel, Internet research and the use of social media

PHYSICAL/MENTAL REQUIREMENTS:

  • Must be able to
    • walk, stand, stoop, reach, twist, turn and lift twenty (20) pounds
    • perform gross motor skills such as carrying items such as books or files
    • perform fine motor skills requiring hand-eye coordination such as keyboard skills
    • see objects closely as in reading case histories
    • hear normal sounds with background noise, distinguish voice patterns and communicate with speech as in using a telephone and making presentations
    • attend to detail and concentrate for sixty (60) minutes or more with some interruptions, remember multiple assignments over long periods of time and understand the theories behind several related concepts
    • perform simple mathematic calculations
    • write effectively, appropriate to the needs of the audience
    • don safety gear such as masks, coats, boots, goggles, gloves or a shield
  • The work environment may involve potential risk exposure to situations or environmental stress that requires a range of safety and other precautions

MINIMUM TRAINING AND EXPERIENCE:

  • Bachelor’s degree in social work; master’s degree preferred
  • Two (2) years’ work experience with seniors, disabled adults and/or financially at-risk individuals and families in a similar setting

LICENSE OR CERTIFICATION:

  • Valid Driver’s License
