What are the responsibilities and job description for the Marketing, Care Transition Liaison LPN position at Traditional Home Health Care?

Shift: Full-time Monday - Friday 8:30 - 5:00
Location: Scranton, PA, Lackawanna County and surrounding areas
Job Responsibilities include but are not limited to the following:
- Establish and maintain collaborative relationships with referring physicians and facilities
- Facilitate referrals of new / existing patients to Traditional Home Health and Hospice
- Meet with patients and ease transition between referring physician / facility to Traditional Home Health or Hospice service
- Obtain appropriate medical medical record information and assist with intake of new patients
- Collaborate with Traditional Home Health and Hospice staff to ensure patients seamlessly transition to our service
- Current license as an LPN in Pennsylvania
- Current Driver's License
- Ability to travel to as needed
- Home Health / Hospice experience
- Exceptional Interpersonal Skills
- Strong written and verbal communication skills
- Ability to prioritize and manage time effectively
