Description
The Admission Registered Nurse is responsible for the initial assessments and all documentation of services for hospice services. The Admission Registered Nurse ensures proper communication between the family, facility, referring individual, attending physician and hospice staff ensuring all required items are obtained and the initial assessment into the service line is a smooth transition.

Duties and Responsibilities
- Completing the patient admissions (including but not limited to consents, admitting paperwork and the physical assessment) across the hospice ensuring proper utilization of services available to the patient.
- Collaborating with staff, facilities, attending physicians, referring individuals and families ensuring that all admissions are scheduled and communicated with appropriate staff members/supervisors, appropriate medications/supplies are ordered (as needed)
- Verify accuracy of data entry from completed admissions ensuring compliance with accreditation, state and federal standards of practice. (Referral Sources, physicians, etc.)
- Maintains a strong working knowledge of the Local Coverage Determinations (LCDs) and can articulate justification for hospice admission to both the hospice and attending physician.
- Maintains current knowledge related to Medicare Conditions of Participation and the Community Health Accreditation Program’s (CHAP) standards to ensure that the organization is compliant with state, federal and accreditation guidelines.
- Additional patient visits as time allows, to assist the hospice team in ensuring quality of care is obtained and maintained.
- Participate in staff meetings, department meetings, team meetings, briefing, in-services, and other related activities as needed
- Assure for compliance with local, state, and federal laws, Medicare regulations and established company policies and procedures.

Requirements
Education and Experience
- Registered Nurse with current licensure
- 3 years of hospice experience
- Certified Hospice & Palliative RN preferred
- Excellent understanding of performance improvement and utilization management.
- Expert level PC skills-Microsoft Word, Excel, and Google products.
- Excellent organization skills required
- Exemplary people skills including both written & verbal communication methods across state and departmental lines.
- Homecare Homebase & Nextgen experience preferred
- Ability to work in an interdisciplinary team
- Compliance with accepted professional standards and practices
Working Conditions
General office, extended care facility, hospice inpatient unit, hospitals, group homes, residential care facilities, patient home setting, exposure to infection diseases, travel by automobile and occasionally by airplane.
Physical Demands
While performing the duties of this job, the employee is regularly required to use hands, talk and hear. Also required to sit, stand, walk and reach with hands and arms, twist, stoop, kneel, crouch or crawl. Occasionally required to lift and/or move items up to 25 lbs. Specific visions abilities include close, peripheral, depth perception, and ability to adjust focus.
Salary : $59,500 - $75,300
