Our Social Workers also have the opportunity to obtain clinical supervision hours as an on-the-job benefit through our Director of Community Care who is a Licensed Independent Clinical Social Worker. 


PURPOSE

Tri-Cities Chaplaincy guides our community members to live their best lives, even and especially through the end of life. We are the recognized source for expert guidance during a crisis, at and through life’s end, and following a loss.


Under the supervision of the Social Work Supervisor or the Director of Community Care, the Hospice Social Worker (MSW) is responsible for the delivery of varied social work services to Hospice patients and their families in homes, facilities, and at Hospice House. This position provides initial biopsychosocial evaluations, ongoing psychosocial support, direct case management services, bereavement services, and community education, outreach, and referral. The Hospice Social Worker (MSW) is an integral part of the Hospice interdisciplinary team.

At Tri-Cities Chaplaincy, we carefully establish salary ranges for our positions by taking various factors into account, including education, certifications, knowledge, skills, experience, and market conditions. These ranges are tailored to match the diverse programs and services we offer. Our goal is to ensure fair compensation for candidates based on these factors while maintaining internal peer equity.


Pay Range:$26.11/hr - $40.15/hr

Starting Pay Range for this Position: $32.56/hr - $38.15/hr


· Mileage Reimbursement: $0.67 per mile, for eligible positions.

· Vacation: Accrual of hours each pay period based on hours worked.

· Wellness Days: After the 90-day introductory period, eligible employees can convert up to two sick days into wellness days.

· Annual "Yay Day": Full-time employees receive 1 paid day off every January to volunteer in the community.

· Sick Time: Earned on an accrual basis, with a 90-day hold for new hires (usable after 90 days)

· Paid Holidays: Ten recognized holidays with no waiting period for new hires.

· Health Benefits: Opt-in for Low Deductible Health Benefits, Vision, and Dental starting the first month following your start date.

· Health Benefit Options: Medical, Dental, Vision, Supplemental Life, Accidental Death and Dismemberment, GlobeLife Voluntary Benefits, and Life Flight.

· Voluntary Supplemental Benefits: Pet Insurance, Globe Life, and Life Flight. 

· Agency-covered benefits: Basic Life Insurance and Accidental Death and Dismemberment at 1x Annual Salary, Long-Term Disability, and Short-Term Disability.

· 401k: You can start contributing right away and up to 5% employer matching after 1 year of employment for eligible employees.

· Premium Pay: Bilingual in Spanish: Verified through ALTA staff testing and Approved Certifications: Premium pay upon review and approval.

· Tuition Assistance: After 1-year of employment, eligible employees can receive up to $5,000 annually in higher education assistance.

· Continuing Education Units: Tri-Cities chaplaincy utilizes Relias Learning and CAPC for employees to earn continuing education units for their credential requirements.

Certification and Training Opportunities: Employees can partner with their department director on professional goals that benefit the agency and themselves for certification and training opportunities 

Requirements

MINIMUM POSITION QUALIFICATIONS

  1. Master's Degree in Social Work from an institution accredited by the Council on Social Work Education.
  2. One (1) year of post-education social work experience in a health care or hospice setting.
  3. Demonstrated ability in case management, counseling, and group work.
  4. Ability to work flexible hours, such as occasional evenings, weekends, holidays, and overtime as needed for patient care and operational needs.
  5. This position is designated as safety-sensitive and will require pre-employment drug testing, which includes cannabis testing in WA State.

PREFERRED POSITION QUALIFICATIONS

  1. Bilingual in Spanish.
  2. Experience in a hospice or healthcare setting.
  3. Experience in assisting clients and patients navigate benefits systems such as Department of Veterans Affairs, Medicaid, COPES, and others.

AGENCY REQUIREMENTS

  1. Demonstrate support and adherence to the agency’s Mission, Vision, and Values.
  2. Participate in community Education and Outreach to raise awareness of agency-provided programs including but not limited to hospice care, advanced care planning, and bereavement.
  3. Responsible for knowledge and compliance with agency policies, procedures, and practices.
  4. Responsible for maintaining and protecting Patient and agency information in a confidential manner. Practice appropriate HIPAA confidentiality by accessing patient-related information on a “need to know” basis only.
  5. Reliable personal transportation, current and valid driver’s license, and vehicle insurance are required.
  6. Satisfaction of state and agency requirements for Tuberculin screening.
  7. Hepatitis B immunization series, proof of series, or signed declination.
  8. Proof of applicable Vaccination(s) on file or approved accommodation/declination as appropriate on file.
  9. Must pass a criminal background screening and pre-employment drug screening.
  10. Ability to meet the physical demands of the position (see attached physical requirements and environmental conditions).
  11. Complete agency and departmental orientation and training program(s).
  12. Complete all continued learning, education, policy reviews, and required training by their respective deadlines.
  13. Maintain licensure(s) and certification(s) required for the position.
  14. Available to work as assigned/scheduled.
  15. Perform other duties as assigned.

RESPONSIBILITIES AND ESSENTIAL FUNCTIONS

  1. Maintains the standards of Social Work excellence in accordance with the principles set forth by the Washington State Department of Health, NASW, NHPCO, and Tri-Cities Chaplaincy policies and procedures.
  2. Presents the Hospice Philosophy to prospective Hospice patients and their families (“informational”).
  3. Coordinates patient intake and assessments with patients, family members, and the interdisciplinary team in patient homes, in facilities, and at Hospice House.
  4. Performs comprehensive initial biopsychosocial and bereavement evaluations and assists in the development and implementation of goal-directed patient care plans.
  5. Assesses a patient’s financial status, as requested by the family, to determine eligibility for services such as Medicaid, COPES, respite care, etc.
  6. Conducts ongoing reassessments of patient/family needs and counseling as needed or requested.
  7. Helps the patient and their family to maintain control and autonomy by empowering them to make their own decisions.
  8. Works from a Systems Theory perspective which includes an understanding of the relationship between the patient, their family, groups, organizations, communities, and other dynamics affecting their environment.
  9. Facilitates family meetings to assist with caregiving needs, family communication issues, and relationship issues.
  10. Assesses factors that may complicate caregiving, such as drug and/or alcohol use or violence in the home.
  11. Provides short-term crisis intervention and individual or family counseling when indicated.
  12. Assists with anticipatory grief issues, providing oral and written resources for family members of all ages and developmental stages.
  13. Participates as a member of the Bereavement Team as assigned.
  14. Observe, evaluate, and bring to team conferences information regarding psychosocial, spiritual, physical, and financial conditions affecting the patient and family.
  15. Assumes the active role of advocate for the patient/family unit.
  16. Documents comprehensive biopsychosocial information clearly and concisely in a timely manner.
  17. Records all initial contact information within 24 hours.
  18. Records all patient/family visits, telephone contacts, and referral actions in the patient’s Clinical Record within a period of 24 hours.
  19. Ensures ongoing sharing of patient and family information between all team disciplines and supervisors.
  20. Attends staff meetings, Interdisciplinary Team meetings, social services meetings, and other meetings as assigned and appropriate.
  21. Exhibits Hospice Philosophy in all job-related roles.
  22. All Tri-Cities Chaplaincy employees will follow the minimum standard regarding PHI when accessing patient information on a need-to-know basis only. All employees will practice appropriate safeguards when accessing this information.
  23. Perform other duties as assigned.
