Job Summary
Provides complex or clinical specialty area care for people living with diabetes in accordance with established protocols and multi-disciplinary plan of care utilizing independent and interdependent interventions to restore stability, prevent complications, and achieve and maintain optimal patient responses through patient-centered / patient-valued care.
Job Duties
Assists in establishing a clear, timely and coordinated approach to the referral process for community-based, ambulatory and /or inpatient Diabetes Self-Management Education and support.
Performs a comprehensive patient assessment, including collection of relevant medical data. Documents in the medical record relevant assessment data, educational plan, education services provided, evaluation results and relevant discharge / support services recommended or coordinated. Documents community diabetes education activities performed as per established processes.
Evaluates effectiveness of Diabetic and education services provided by measuring attainment of learning objectives, through patient / family demonstration and / or verbalization. Participate in the continuous quality improvement processes to measure and to identify and address opportunities for improvement.
Collaboratively (patient, multidisciplinary team and referring provider) develops educational goals, learning objectives, educational content and teaching methods, including an individualized support plan, for all patients referred for Diabetic education services and where relevant, for diabetes education activity conducted in the community. Provides educational interventions that utilize interactive, collaborative, skill-based training methods.
Serves as a patient / community advocate, by focusing on their needs, rights, confidentiality, and religious and cultural preferences. Ensures positive working relationships are maintained with all internal and external customers, employees and community partners etc. Serves as a resource for patients, physicians, other healthcare team members and community partners / support personnel.
Strives to advocate for and implement the American Diabetes Association Standards of Medical Care in Diabetes and the National Standards for Diabetes Self-Management Education and Support to assist in building the structure to deliver consistent, best-practice care across the health system and community to achieve better outcomes.
May lead and / or participate in community outreach activities (such as health fairs, community collaborations or outreach clinics) as assigned. Serves on diabetes-related committees and completes special projects, including possible community projects, as assigned.
Follows Tribal Health and client specific policy and procedures.
All other duties as assigned and specified by clinical assignment and site.
Requirements:

Certifications required
- BLS
- Diabetic Educator Certification preferred or required to be obtained within 2 years of employment

Experience requirements
- 3 years
- Excellent verbal, written and interpersonal communication skills a must.
- Possess a quality “bed side manner”.
- Able to maintain a professional working relationship with community and staff.
- Must have basic computer skills and working knowledge of EHR.
- Maintain strict confidentiality.
- Pass pre-employment drug screen and complete background investigation.
- Able to lift 40 lbs or more.

Duties
- Collaborate with primary care providers to develop individual diabetic treatment plans and set patient goals.
- Provide education awareness to individuals and the community in areas of diabetic disease process, secondary body system complications, blood sugar maintenance, medication compliance, insulin delivery, foot care, ulcer care, making positive lifestyle changes, improving nutrition, and increasing physical activity.
- Perform or schedule patient health screenings as appropriate to identify and treat secondary complications related to diabetes per IHS guidelines.
- Coordinate ongoing community diabetes education, community outreach, home base services and prevention activities in collaboration with the Nutrition Education Coordinator and other Clinic/Tribal staff.
- Document patient contacts in the Electronic Health Records (EHR) system.
- Collaborate with nutrition and wellness staff to develop individual nutrition and fitness routines for better disease control.
- Maintain diabetic registry in the S.O.S/EHR system.
- Develop registry of pre-diabetic clients in the EHR system.
- Conduct outreach to pre-diabetic community members to engage them in prevention activities.
- Complete monthly report of program activities and outcomes for the Director.
- Compile data and submit Annual Diabetic Audit to IHS.
- Intake and screen patients; document the patient’s chief complaint(s), patient and family medical history, vital signs, symptoms, medications in the EHR system and communicate with primary care provider.
- Assist with exams and treatment for patients/families with wide variety of complex medical problems in all age groups.
- Assist in coordination of patient referrals for specialty care.
- Perform other duties as assigned
