Job Summary
The ISNP (Institutional Special Needs Plan) Coordinator plays a pivotal role in the business development process by establishing and nurturing relationships with Senior Living Facilities and Nursing Homes to introduce the affiliation with Provider Partners Health Plan of Indiana, Inc.
ISNP & IESNP. This position is responsible for identifying and qualifying eligible Senior Living and Nursing Home residents, gathering essential information, reaching out to the residents or Responsible Parties (RPs) to make them aware of the affiliation with Provider Partners and offer the eligible resident or RP the opportunity to meet with a Provider Partners Benefits Advisor.
Roles and Responsibilities
- Establishes and maintains strong relationships with key contacts at participating facilities.
- Identifies residents who meet eligibility criteria for the plan routinely.
- Presents the affiliation information and offers the resident or RP the opportunity to meet with a Provider Partners Benefits Advisor, routinely ensuring they have access to continuous support.
- Tracks and Maintains accurate roster of enrollees.
- Conducts thorough research to gather contact information for residents or RPs and initiate initial contact.
- Ensures all activities comply with relevant regulations and industry standards, especially in healthcare-related services.

- Tracks and Trends key quality metrics associated with plan performance.
- Other duties as assigned.
Qualifications
Education : Bachelor Degree
Experience : 1-3 years
Physical Requirements
Sitting, standing, bending, reaching, stretching, stooping, walking, and moving intermittently during working hours. Must be able to lift at least 50lbs.
Must be able to maintain verbal and written communication with co-workers, supervisors, residents, family members, visitors, vendors, and all business associates outside of the health campus.
