Case Manager

Duties: - Conduct comprehensive assessments of clients' needs and develop individualized care plans - Coordinate and monitor services to ensure clients receive appropriate support and resources - Provide crisis intervention and support during times of emergency or high-stress situations - Collaborate with other professionals, such as social workers, healthcare providers, and educators, to ensure holistic care for clients - Advocate for clients' rights and access to necessary services - Monitor client progress and adjust care plans as needed - Provide mentoring and guidance to clients in areas such as childcare, workforce development, and behavioral health - Utilize motivational interviewing techniques to empower clients in achieving their goals - Maintain accurate and up-to-date documentation of client interactions and progress Requirements: - Bachelor's degree in social work or related field (Master's degree preferred) - Previous experience in case management or related field - Knowledge of child welfare systems, Medicare regulations, and workforce development programs - Strong understanding of crisis management techniques - Excellent communication skills, both written and verbal - Ability to work independently as well as collaboratively within a team - Proficient in using computer software for documentation purposes - Strong organizational skills with the ability to prioritize tasks effectively - Ability to supervise and provide guidance to other staff members We offer competitive compensation packages along with benefits such as healthcare coverage, retirement plans, paid time off, and professional development opportunities. Pay: $37,262.00 - $37,888.00 per year
