Full Time, On-Call position, Thursday 3 pm-11 pm, Friday 3 pm-11 pm, Saturday 7 am-11 pm, and every other Wednesday from 3 pm-11pm.a. Full Time status (72 hours per Pay Period)

c. Will have some scheduled visits during the on-call timeframe
d. Will get paid hourly for the entire shift even when you don’t have an assignment
e. Hourly rate is $15.00 per hour plus $3/hr shift differential
POSITION TITLE: Home Health Aide
POSITION SUMMARY:
Provides patient care based on an established home health aide care plan. Duties include assistance with activities of daily living, patient monitoring as defined in the home health aide plan of care, light housekeeping and assistance with an established exercise program.
EDUCATION, TRAINING, EXPERIENCE, CERTIFICATION AND LICENSURE:
High school graduate or equivalent. Twelve hours of approved in-service per year required. Valid NYS Driver's license. Holds a Home Health Aide Certificate or Certified Nursing Assistant Certificate from an approved NYS training program or successfully completes an approved training/equivalency program which is provided if offered a position. Participates in orientation and continuing education and updates and maintains knowledge and skills related to specific areas of expertise. Completes population specific competency annually on populations served as identified in scope of care and service.
SPECIAL EQUIPMENT, SKILLS OR OTHER REQUIREMENTS:
Demonstrates an ability to communicate effectively, read, write and carry out directions. Demonstrates a sympathetic attitude towards the care of the sick. Successfully completes a competency review. Completes a criminal background check.
WORK ENVIRONMENT AND HAZARDS:
Home setting. Travel daily. Intermittent to regular exposure to unpleasant physical and environmental conditions. Occasional to regular exposure to situations that could cause moderate to serious illness or injury. Exposure Class I.
PHYSICAL DEMANDS:
Light work: lifting, sitting, standing. Close visual attention three to four hours per day.

Patients, employees, supervisors, schedulers, case managers and various regulatory and professional agency staff.
