JOB SUMMARY
The OB Care Coordinator works with patients to ensure the highest quality of services is provided. They must provide patients and their families with educational information in conjunction with direct care providers related to treatment, procedures, medications, and continuing care requirements. They work with administration, staff, and patients to reach healthcare goals and keep the lines of communication open. Responsible for coordinating with the obstetrics department by ensuring that all the steps necessary for prenatal care and postpartum care, and follow-up are functional and ongoing.
PRIMARY FUNCTIONS
- Plan with the patient, family/caregiver, primary care provider, support staff, payers, and community resources, to maximize healthcare responses, quality, and cost-effective outcomes.
- Facilitate communication and coordination between healthcare team members and involve the patient in decision-making to minimize service fragmentation.
- Educate the patient, family or caregiver, and members of the healthcare delivery team about treatment options, community resources, insurance benefits, psychosocial concerns, care coordination, etc., so that timely and informed decisions can be made.
- Empower the patient to problem-solve by exploring options of care when available and alternative plans, when necessary, to achieve desired outcomes.
- Encourage the appropriate use of healthcare services, strive to improve the quality of care, and maintain cost effectiveness on a case-by-case basis.
- Assist the patient in safely transitioning care to the next most appropriate level.
- Strive to promote patient self-advocacy and self-determination.
- Advocate for the patient to facilitate positive outcomes for the patient, health care team, and payers.
- Attend monthly meetings with insurance payers to have a consistent line of communication open and assist with setting goals for quality improvement.
- Stay involved and active with quality improvement activities.
- Assist the referral department with referral tracking: including documentation, phone calls, and sending out reminder letters of patients’ pending referrals.
- Complete outreach calls to close open HEDIS measures for obstetric patients
- Works closely with the obstetrics department to ensure the patient receives timely care.
- Review the patient chart to ensure the OB episode is accurate and current.
- Assist with patient scheduling after delivery for postpartum follow-up appointments.
- Works closely with hospital staff to ensure timely follow-up.
- Comply with the department goals set for the OB Care Coordinator.
- Conduct quality of care audits as required.
- Assist with data entry for tracking purposes (i.e., UDS reporting, Notice of Pregnancy (NOP)
- Other duties as assigned.
EDUCATION AND EXPERIENCE
- High school diploma or equivalent is required.
- Associate or bachelor’s degree in healthcare administration or related area field is preferred.
- Medical Assistant certification is preferred.
- One (1) year minimum experience in scheduling/care coordination/case management

ADDITIONAL QUALIFICATIONS
- Bilingual is a plus (English / Spanish)

- Ability to stand and or walk for extended periods of time

- Ability to communicate effectively with patients and staff
