What are the responsibilities and job description for the Home Health Clinicians position at Trust USA Home Health?
Home Health RNs, LPNs, and Home Health Aides needed though out Broward and Palm Beach County! Trust USA Home Health and All About You Home Health service areas from Broward County to Martin County/Port Saint Lucie. Home Health RN Duties:
- Making home visits in designated area to perform initial and ongoing assessments/treatments for patients
- Performing assigned duties including administration of medication, wound care, treatments and procedures
- Identify and communicate changes in the patient's condition
- Maintain communication with patient, family and other members of the home care team to ensure coordination of services and meet the patient goals
- Executing and properly documenting the doctor's orders
- Prepare, submit and maintain documentation as required by the company. Visit/shift notes documented on day services are rendered
Home Health LPN Duties:
- Provide skilled nursing interventions in the treatment of the patients’ illness, rehabilitative needs and preventative care
- Apply knowledge and skills in accordance with accepted standards of clinical practice to facilitate problem resolution and achieve individualized patient goals and outcomes
- Confer with the RN Supervisor regarding needed changes in the Plan of Treatment. Accept verbal orders from physician were permitted by state law/regulations/Nurse Practice Act and communicate these orders to the RN Supervisor
- Prepare, submit and maintain documentation as required by the company. Visit/shift notes documented on day services are rendered
Home Health Aide Duties:
- Provide health care services in patients' residences
- Perform domestic and household tasks
- Administer simple prescribed medications
- Assist with patients’ personal care activities
- Monitor patients (vital signs, temperature, respiration, etc.) and report on their condition
- Maintain patients care records and document provided services
- Assist patients with mobility and physical therapies/exercises
All clinicians must have:
- At least 2 years experience in home health
- Experience using Oasis EMR
- Active licenses
- Up to date certifications
- Up to date BLS/CPR
- COVID vaccine
Rates are per visit and negotiable. Your schedule is based upon your availability; full-time, part-time or per diem.
Job Types: Full-time, Part-time, Contract
Pay: $35,000.00 - $75,000.00 per year
