**Experience: • 24 months of Home Health experience is required. • 3 months of travel experience is required. • 3 months of experience with Epic is preferred. Requirements: • Candidates must have a New Mexico license or compact license in hand at the time of application. • This role will require floating to multiple locations • Candidates must live at least 75 miles away from this facility in order to be eligible to apply for this role. • COVID vaccination required for onboarding. Religious and medical declinations accepted. • COVID booster required for onboarding. Religious, medical, and personal declinations accepted. • Flu vaccination required for onboarding. Religious and medical declinations accepted. Pre-employment modules may be required for this role. Additional Details: Travelers must have a permanent address outside of the 75 mile straight line radius of intended facility, not driving distance. Guaranteed Hours: 36 NO LOCAL TRAVELERS Please note time off requests must be disclosed with initial submission to be considered for approval. Time off requested after the acceptance date will not be approved. Must have successfully completed at least one 13-week travel assignment. SHIFTS: 8 hours (or plus) varied - starts at 8am - May fluctuate to fulfill department staffing needs. One 8 hour on call shift required per 6 weeks. Weekday and rotate weekends and holidays may be required. Traveling pairs are not guaranteed same schedules or same shifts. Approximately 5 to 6 home visits per day. Travelers must not have been an employee of the Presbyterian Healthcare Services system for 12 months from last day worked, in order to be considered for a travel position Travelers are first to float within facility and to alternate locations within 20 miles of permanent assigned location. 1 reference from last 12 months (Any reference type)1 reference from last 12 months (Manager/Supervisor reference type) Certifications: • BLS (Basic Life Support)
