This is a 40 hour per week position with our College Testing Program.
GENERAL SUMMARY
The Licensed Practical Nurse (LPN) provides care to patients and is responsible for patient care as assigned by a licensed independent provider (MD, NP, PA, RN). The LPN participates in direct and indirect nursing care, health maintenance, patient instruction, collaborative planning, and rehabilitations within scope of practice and consistent with his/her education and training. Developing relationships with patient’s families and colleagues, the LPN functions as an interdisciplinary team member who provides quality patient care and contributes towards the attainment of important goals for the patient, practice, and organization.
PRINCIPAL DUTIES AND ESSENTIAL FUNCTIONS
Clinical Practice and Responsibilities
- Provides direct patient care within the scope of his/her preparation and within current standards of nursing practice. In conjunction with the licensed independent provider or RN, participates in the assessment, planning, and evaluation of the nursing care delivered.
- Delivers care to patients in a safe efficient manner maintaining the patients rights and dignity at all times.
- Demonstrates engagement and enthusiasm to provide nursing care to patients and establishes a positive relationship with the patient and family.
- Collaborates as a member of the health care team, contributing toward a supportive and therapeutic environment that promotes the attainment of patient goals that are specific measurable, action oriented, realistic and timely.
- Accurately obtains vital signs, elicits information about a patients’ basic health status, administers relevant nursing care within scope of practice and as directed by the Licensed Independent Provider (LIP)/Registered Nurse (RN);documents all nursing interventions and the patient’s response in the electronic medical record for all assigned patients.

JOB DESCRIPTION
- Performs routine tests that may include but not limited to: EKG, vision and hearing screening, and the collection of specimens for laboratory and point of care testing.
- Administers medications as determined by the LIP and according to established policies and procedures.
- Participates in team based care and patient education initiatives to support the health and wellness of patients in conjunction with the LIP/RN. Offers basic instruction per protocol or as directed. Reinforces patient instruction as provided by LIP/RN.
- Participates in organization and unit based quality improvement initiatives and compliance readiness
- Participates in telephone screening of patient calls; may provide instruction to patient when following approved protocol or as directed by the LIP/RN.
- Participates in prescription management tasks including the preparation of new prescriptions and renewals requiring LIP approval, prior authorizations and medication reconciliation.
- Appropriately assigns patient care to others within the scope of his/her practice and maintains responsibility for the optimal outcomes of such care.
- May preform other duties as assigned such as but not limited to; flow, stocking of rooms, scheduling tasks, and all other duties as assigned.

Qualifications
JOB KNOWLEDGE AND SKILLS
- Possesses and applies the skills and knowledge necessary to provide care to patients throughout the life span, with consideration of aging process, human development stages and cultural patterns in each step of the care process.
- Ability to provide care for a designated group of patients according to the unit guidelines and hospital policies, procedures and protocols.
- Basic knowledge of patient education
- Ability to perform vital signs, administer medication within scope, injections, and oral medications
- Ability to perform basic math for safe medication administration, ability to participate in medication reconciliation
- Proficient communication, writing, phone, and organization, and electronic system skills required for this role.

EDUCATION
- Graduate of an accredited Licensed Practical Nursing Program and High School Diploma or equivalent required
EXPERIENCE:
- LPN Experience in a health care setting
- Experience in an outpatient/ambulatory setting preferred
- Knowledge of practice operations and standards, medical terminology
