STATEMENT OF WORK
Minimum Qualifications:
- Requires current Pennsylvania License for Practical Nurse or is eligible to sit for the Practical Nurse Licensure Examination.
- Proficient computer skills to include, but not limited to, Microsoft Office products.
- Ability to enter health data for residents into electronic medical records data base.
CONTRACT SCOPE/OVERVIEW
This contract will cover the requirements of Licensed Practical Nurses (LPNs) with the Department of Human Services (DHS) / Danville State Hospital (DSH).
LOCATION
The location is Danville State Hospital, 50 Kirkbride Drive, Danville, Pa. 17821. The patient population consists of adult psychiatric patients at least 18 years of age. DSH has a bed capacity of 161 consumers.
SERVICE/HOURS
DSH maintains 3 shifts (6:45am – 3:15pm, 2:45pm – 11:15pm, and 10:45pm – 7:15am) for LPNs. The selected resource(s) will be assigned based on the needs of DSH. A ½ hour (30 min) unpaid lunch and two 15 minute breaks will be given to the resource(s) performing duties under this contract. Time of break and lunch is dependent on shift assignment and DSH needs.
MANDATORY TRAINING
All contract staff is required to attend orientation for a period as determined by DSH and will be required to attend training during the normal day time business hours. Training will be provided by DSH staff. Successful completion of training is mandatory.
PERFORMANCE STANDARDS
The hospital reserves the right to reject or immediately remove any unqualified contract staff if they cannot perform their duties at an acceptable level in order to meet the hospital’s standards of care and practice. The supplier is expected to replace the contract staff ASAP depending on the needs of DSH.
The Hospital will provide all contract staff with an Identification Badge, parking permit, and set of keys.
Each shall be required to sign a statement regarding confidentiality of consumer records and guaranteeing consumer rights.
DRESS CODE
ID badge must be worn at all times when on duty and must be clearly visible and readable to other staff and consumers. Licensed Practical Nurses (LPNs) shall dress in a responsible manner that displays professionalism at all times. DSH shall instruct the resource on the dress code.
SIGN IN/OUT
All contractors must sign in and out at our Communications Center and have the staff there verify by also signing. On the rare occasion that Security is available; the contractor will need to seek the Nursing Supervisor on duty.
Special Treatment Procedures
- Utilizes current crisis prevention techniques (when trained).
- Ensures the use of least restrictive measures and follows Hospital policies/procedures in the care of consumer(s) in restraint/special observation.
Emergency/Medical Interventions/Use of Medical Equipment
- Adheres to Infection Control / Universal Precautions policies and procedures.
- Responds to medical emergencies, takes appropriate actions for which specifically trained. These could include cardio-pulmonary resuscitation (CPR), first aid, foreign body airway obstruction management, or other emergency treatment procedures within the LPN scope of practice. Following emergency, takes appropriate action according to DSH Policies and Procedures.
- Responds to disaster / emergencies, e.g., fires, earthquakes, floods, hazardous materials incidents, bomb threats, utility interruptions or transportation accidents; follows basic emergency procedures as contained in DSH Policies and Procedures.
Competency Maintenance Activities
- Attends and participates in ongoing mandatory DHS, hospital and departmental training programs.
- Attends in-service training programs.
Oral and Written Communications
- Interacts with consumers utilizing therapeutic communication skills.
- Writes/prints legibly.
- Documents specific, accurate and complete information in the Medical Record.
- Documents in DAR format.
- Reports factual patient information.
- Provides accurate and legible written work/incident related statements as required. Reports/documents consumer incidents. Complies with documentation policies and procedures for the completion of all hospital reports.
- Maintains confidentiality of consumer information.
- Report’s problems accurately using the chain of command.
- Keeps conversations free from discriminatory, abusive or sexually harassing comments.
