JOB DESCRIPTION

Medication Assisted Treatment (MAT) Social Worker Responsibilities:

  1. The MAT Social Worker shall be responsible for working with offenders by promoting, counseling, and providing consultation and documentation regarding interest in MAT.
  2. The MAT Social Worker shall ensure that those interested in the use of medication assisted treatment shall have opportunity to participate if the offender meets the criteria.
  3. The Social Worker shall follow the direction provided by the Division of Treatment Services (BTS), with regards to the MAT program.

Requirements:

  1. The Social Worker shall adhere to all contract and policy stipulations.
  2. The Social Worker shall subscribe to the AOD treatment framework (MET/CBT) and the policies and procedures of the DOC.
  3. The Social Worker shall implement and provide social work, counseling, and case management services to individuals to enhance their social functioning and to help them attain a more satisfactory social, economic, emotional, or physical adjustment.
  4. The Social Worker shall perform in accordance with applicable rules, regulations, policies, and procedures under the general supervision of an administrative supervisor.
  5. The Social Worker shall assess offenders’ psychosocial strengths, weaknesses, and social work service’s needs, and eligibility for services.
  6. The Social Worker shall assist individuals in recognizing problems and exploring methods for adjusting, modifying, and improving personal and social behavior.
  7. The Social Worker shall discuss assigned individuals’ psychosocial assessments and family history in multidisciplinary treatment team meetings.
  8. The Social Worker shall ensure needs, benefits, and services are obtained in accordance with assigned individual’s treatment plans.
  9. The Social Worker shall be responsible for the records, updates, and maintenance of assigned case management files on individual’s progress in meeting treatment plan goals and objectives.
  10. The Social Worker shall confer with facility staff and external agency representatives on discharge plans.
  11. The Social Worker shall write social services discharge/case closing/transfer plans summarizing individual’s disposition and the need for continued aftercare or follow up services upon discharge.
