Telehealth Patient Facilitator
Minimum Qualifications:
- Knowledge of Telehealth processes and procedures is preferred.
- Background in mental health or physical health preferred.
- Previous administrative and/or medical knowledge required
- CNA or MA is a plus
Role Description:
- Retrieve any necessary forms and paperwork needed by the Telehealth Provider.
- Prepare treatment rooms for appointment.
- In the absence of electronic health records, fax/scan/email most recent progress note and any other relevant documentation to the Provider prior to the session as well as receive notes and relevant orders from the Provider at the conclusion of the session.
- Ensure completion of all required forms.
- When applicable coordinate with scheduler for scheduling appointments.
- Monitor daily clinic “no shows” and coordinate with security to facilitate patient arrival; adjust clinic times in response to lockdowns, count, etc.
- Assist in orientation and training of new Telehealth providers including locums regarding facility operations and procedures.
- Educate the patient on the nature and process of Telehealth, specifically discussing confidentiality.
- Obtain patient consent when required.
- Ensure all equipment is functional and fully operational.
- Assist with flow of patients in and out of Telehealth clinic and remain present during clinic session.
- Navigate and review information requested from the chart and share such information with the Provider.
- Assist the provider in using Telehealth equipment.
- Fax/email relevant documentation to the Provider as well as receive notes and relevant orders from the Provider.
- Ensure the call has been disconnected and the power is turned off to the video conferencing equipment at the conclusion of Telehealth session.
- Ensure that notes and pharmacy orders are provided to the Nurse for processing at the conclusion of the appointment; and/or file such notes and orders where appropriate.
- May be required to train other staff regarding the role of the patient facilitator.
