GENERAL PURPOSE

Under the administrative supervision of the Program Director or designee, this position is responsible for completing initial and annual assessments, providing psychosocial, rehabilitation and recovery-oriented services for adults.

DISTINGUISHING CHARACTERISTICS

This is an at-will direct service position within a program. This position is responsible for ensuring that clients are introduced into the program in a comfortable, thorough and timely manner. This position reviews the initial assessment of clients to confirm eligibility under target population and geographical boundary criteria. This position is also responsible for ensuring that annual re-assessments are completed in a strengths-based, client-centered manner and that the assessment and treatment plan are shared with the multi-disciplinary team working with the client.

ESSENTIAL DUTIES AND RESPONSIBILITIES – (ILLUSTRATIVE ONLY)

  • Responsible for assisting members in identifying, expressing, asserting and communicating their goals and needs while living in the community
  • Responsible for completing the intake and renewal Core Assessment, including collateral assessments (AHQ, MSE) in the field or in the office in coordination with the client’s multi-disciplinary team
  • Responsible for facilitating the development of the Individual Client Plan in collaboration with the member and the treatment team
  • Provides on-going consultation to the treatment team and other staff to support the delivery of services that are driven by the member’s client plan and supportive of the member’s goals
  • Completes all required paperwork to include annual member paperwork as assigned in a timely manner
  • Drives on program business using personal vehicle for the purposes of outreach and engagement, assessments, medication outreach, transporting clients to appointments/activities, community/county meetings/trainings
  • Provides prompt intervention in the event of a crisis both to stop the crisis and to notify, when indicated, persons and agencies necessary for the resolution of the crisis situation
  • Provides “on-the-spot” counseling that is both helpful to the clients and consistent with the philosophy of the program
  • Participates in off-site trainings and provides in-service training for all staff
  • Conducts Quality Management activities as directed by the Clinical Director and Team Leader, such as preparation for county Utilization and Review audits, internal audits, review of services provision and charting, and other projects as assigned
  • Facilitates groups, provides individual and family therapy, and updates diagnoses for clients not receiving medication management services from the program psychiatrist, with oversight from a licensed professional if not themselves licensed
  • Carries a small caseload
  • Attends to the safety, health, and wellbeing of clients
  • Completes paperwork as assigned in a timely manner, and maintains productivity standards outlined for the program
  • Provides after hours, “on-call” coverage on a rotational basis to program clients

MINIMUM QUALIFICATIONS AND CERTIFICATIONS & LICENSES

Education, Training and Experience

A typical way of obtaining the knowledge, skills and abilities outlined above is through graduation from a four-year college or university with a bachelor’s degree in psychology, Social Work or related field, and a minimum of four years’ experience as a provider of mental health services; preferably working with ALL demographics, populations, and ages (TAY, adults, and older adults). Experience in conducting functional assessments, bio psychosocial assessment, and other clinical approaches in interviewing and gathering data based on mental health needs, diagnosis, and functional impairments. Basic computer literacy is essential.

Licenses; Certificates; Special Requirements:

  • California driver’s license & current vehicle insurance/registration
  • Reliable means of transportation capable of passing vehicle safety inspection if more than five years old excluding all modes of two-wheeled transport inclusive of bicycles, mopeds and motorcycles.

Schedule: Sunday-Thursday, 12:00 AM - 8:30 AM
