We take a coordinated and collaborative approach to keeping residents healthier longer and safer in their communities, helping to avoid unnecessary ER and hospital visits. Our goal is to alleviate some of the burden and stress senior communities and families feel trying to coordinate needed medical services for residents and their loved ones.
Position Summary: Twin Cities Physicians is seeking a Nurse Practitioner to join our growing care team! With Twin Cities Physicians, you will provide the best care for residents within all types of senior living communities. We bring primary care right to where our patients call home. Responsibilities:
- Drive to Senior Living Communities to provide on-site primary care to residents
- Facilitate the patient's plan of care: chronic care management, advanced treatment, and end-of-life planning
- Coordinate with the interdisciplinary team, reconcile medications, and order labs
- Communicate clearly and regularly with Physician, office support staff, community staff, and patients' families
Qualifications:
- Licensed as a Certified Nurse Practitioner / CNP in the State of Minnesota
- Experience in Geriatrics and/or Primary Care
- Valid driver’s license and current auto insurance
- Passion for serving the senior population
- Strong interpersonal communication skills
