Mobile Crisis Social Worker, Second Shift, Mobile Crisis UnitUC Health is hiring a part-time Mobile Crisis Social Worker for the mobile crisis unit for the second shift at University of Cincinnati Medical Center.The Mobile Crisis Social Worker serving in a variety of departments and clinics is responsible for responding to referrals from the interdisciplinary team. The social worker will complete psychosocial assessments, develop, and implement a plan of care that addresses patients' needs that promotes continuity of care, and facilitates transitions to the next level of care.
