The pay range for this position is: $38.29 - $59.35 / hour. Pay is dependent on applicant's relevant experience.

Responsibilities:
- Coordinates transplant team activities with patients, families, organ donor networks, regulatory agencies, and transplant networks and agencies.
- Coordinates care of liver transplant patients through collaboration with hepatologists, surgeons, APPs and staff.
- Coordinates plan of care with the transplant clinic, dietician, pharmacist, social work and financial coordinator through the continuum of pre-transplant process.
- Documents per policies, procedures and regulatory requirements.
- Participates in notification plans for transplant teams. Managing, monitoring and triaging the care of pre-liver transplant patients. Assists in reviewing and maintaining records of the evaluation information on transplant patients.
- Education of the transplant process to patients and family/friends. May serve in on-call capacity.
- Within scope of job, requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a fast-paced environment and take appropriate action.
Requirements:
- Minimum Required Education: Graduate of an accredited or state board of nursing approved Registered/Professional Nursing program if less than 3 years experience. Preferred: Bachelor's degree in Nursing from an accredited nursing program.
- Required Licensure/Certification: State licensure as a Registered Nurse (RN). Basic Life Support (BLS) Healthcare Provider and any relevant certification as determined at position level.
- Minimum Experience: 2 years of experience in a related field of specialty. Preferred: Transplant coordination experience. Clinical care of transplant recipients.
