We have a new part time pharmacist position available in our clinic at 1300 Woodland Avenue, West Des Moines, Iowa. UCS Healthcare is a CARF-accredited, non-profit health care organization based in West Des Moines, Iowa with additional offices in Knoxville and Ankeny and 11 medication units across the state offering medication assisted treatment (MAT) for opiate/alcohol addiction.
This role is primarily responsible for dispensing prescribed Medications. Operates in alignment with UCS Healthcare core values. Is a team player, provides patient centered services and strives to provide the highest level of care.
Duties, Including but are not limited to:
- Follow all Federal, State of Iowa, and agency policies and laws, including those pertaining to patient confidentiality, including HIPAA/ 42CFR part 2 and CARF requirements.
- Promotes the safe and effective practice of pharmacy, ensuring quality patient care.
- Preparing and dispensing physician prescribed doses of medications. This includes the preparation of "take home doses" (described as "Take Out doses" or "TOs" in Methasoft.)
- Using the Methasoft software to dose patients, control and monitor inventory, and generate and utilize reports in order to aid in the monitoring and treatment of MAT patients.
- Ordering medications through our preferred wholesaler when pharmacy stock is depleted when assigned by PIC.
- Providing pharmaceutical counseling/education to patients and staff.
- Consulting with clinic physicians and staff on pharmacology/pharmacokinetic questions concerning agency patients.
- Utilizing PMP to assess use/misuse of medications or drug seeking behaviors and report to nurse for evaluation of patient records.
- Ensure patient photographs are up to date in Methasoft; take photographs at intake and as necessary.
- Maintaining the cleanliness and professional appearance of the pharmacy.
- Maintaining and cleaning the LabTec methadone pump.
- Counting patient's controlled substance medication to monitor for overuse/abuse when deemed necessary by the pharmacist, nurse, or physician, this will take place outside the pharmacy
- Check patient take home medications for tampering when presented for call back.
- Observing patients for signs of impairment prior to dosing, then referring the patient back to the nurse if appropriate.
- Maintaining records of patient orders and guest dosing requests.
- Maintain schedule in Carelogic and adjust as necessary
- Accurately document patient notes in EHR.
- Arrive for scheduled shifts on time.
- Notify PIC one hours prior to the start of your shift if unable to work.
- Participate in all staff meetings unless absence approved by supervisor.
- Maintain appropriate License and CEU's
- Provide peer recovery services (if certified) when deemed appropriate by a Peer Recovery Coach Supervisor.
- Pharmacy billing as deemed necessary by management.
- Other duties as assigned by management.
Preferred Education and Experience:
- B.S. or PharmD in Pharmacy
- License to practice in the State of Iowa
Preferred Knowledge, Abilities and Skills:
- Maintain a professional environment and continually strive to raise the level of professional practice.
- Interpret physicians' orders as they relate to the patient's drug therapy.
- Evaluate the appropriateness of medication orders based on clinical data available in the medical record (including computerized information) and provide recommendations to physicians when appropriate.
- Monitor adherence to dosing guidelines and target drug protocols and intervene as necessary
- Contact physicians and nurses to clarify orders or recommend changes in therapy as appropriate
- Participate in interdisciplinary team meetings.
- Participate in pharmacy quality management activities.
- Understand and comply with all state and federal pharmacy laws and regulations.
Work Schedule: Part time variable hours (24) Monday - Friday and every third Saturday 6-2.
PRN hours also available.
