JOB SUMMARY

Incumbent will serve as Licensed Vocational Nurse (LVN) / Wellness Coach for Assisted Outpatient Treatment (AOT) services and subsequent Assertive Community Treatment (ACT) for Laura’s Law/AB 1421 eligible adults with serious mental illness in Contra Costa County. The LVN / Wellness Coach provides case management for a shared group of clients including coordinating and monitoring the activities of the treatment plan. The incumbent will complete and oversee assignments to improve the diet, nutrition, and wellness of the clients. The range of authority and decision making for this position is limited to and consistent with MHS Policy for Nursing staff.

  • Provides service coordination for a shared group of clients. Assumes responsibility for monitoring overall treatment goals in conjunction with the treatment team. The majority of AOT ACT services are provided in the community and client residences and requires client transportation in personal and/or company vehicles. Provides on-call, after-hours, and weekend/holiday services as required by contract.
  • This position is designated to develop curriculum, and activities that promote health and wellness.
  • Assists clients in accessing and transitioning to primary care when indicated.
  • Provides individual and group wellness coaching and education. Maintain the distinction between therapeutic and social relationships.
  • Coordinates with primary physicians and health care personnel to determine physical capabilities and nutritional needs. Conducts health and wellness screenings for assigned clients.
  • Develops curriculum and prepares manuals, visual aids, course outlines, and other materials used in promoting health.
  • Works with Program Psychiatrist, nursing staff, case managers and other team staff to monitor the health and well-being of clients.
  • Counsels clients on basic rules of good nutrition, promotes physical activity and healthy choices to improve their quality of life.
  • Collects psychiatric history; mental status; medical and dental history.
  • Dispenses and accounts for all prescribed medication. Responds appropriately to medical emergencies.
  • Provides individual supportive counseling and symptom management. Educates and support clients and family members.
  • In collaboration with the treatment team, participates in the organization and management of medication coaching and delivery to clients, as well as integrating medication administration into treatment plans.
  • Assists in administering Patient Assistance Program, ensuring efficiency and accuracy in all aspects.
  • Completes all charting and documentation as contractually required and in a timely manner as dictated by program
  • requirements.
  • Ensures that all medical policy and procedures are followed and meets regularly with Program Leadership to monitor
  • this. Assists Psychiatrist in administering and evaluating medications prescribed by Psychiatrist, evaluating medication
  • effectiveness, side effects, and complications; and arranging for lab work according to protocol.
  • Participates in and represents program at Medication Monitoring Committee Meetings; acts as liaison with Program Psychiatrist to obtain corrections to documentation and adjustments to procedures as needed.
  • Introduces clients to Common Ground software, assisting clients in completing the Common Ground Health Report before medical office visits.
  • Successfully functions as a team member, supporting the team in providing case management and rehabilitation visits when needed and when directed by Program Leadership.
  • Serves in a mentoring role to staff and interns re: pharmacology and patient care.
  • Working alongside the RN to coordinate medication support throughout the week. 
  • Comply with all MHS and program rules, policies and procedures including HIPAA standards.

                

QUALIFICATIONS

  • Current licensure as a Vocational Nurse in California
  • Minimum of three years of experience treating psychiatric and dually diagnosed population
  • Demonstrated knowledge of cultural and socio-economic factors that inform service provision to persons with psychiatric or dual diagnosis disorders; demonstrated ability to engage culturally diverse populations 
  • Ability to create written documentation that is legible, clear, and concise
  • Valid California driver’s license and current auto insurance coverage 
  • Proficient with Microsoft Office Suite and EHR software
  • Ability to work independently and effectively collaborate as part of a team
  • Ability to problem solve and react appropriately to emergency situations
