What are the responsibilities and job description for the Case Manager - PRN position at UHS?
Position Summary:
The Case Manager is responsible for meeting with all eligible adult patients at Cedar Ridge at Bethany to discuss the benefits of stepdown services as part of their continuum of care treatment. Will be responsible for communicating eligibility to key members of senior leadership and associated departments and will be responsible for routinely collecting and entering data, specific to eligibility, into Midas for trending report purposes. The navigator will have a thorough understanding of the step-down process and will coordinate between departments. Assists therapists with discharge process, develops referral sources, and performs duties in accordance with the ANA Standards of Practice for Psychiatric Mental Health Care, and facility mission, values, policies, and behavioral standards.

Cedar Ridge Behavioral Hospital at Bethany is a 56-bed psychiatric hospital offering adult behavioral health programs and treatment services. Our team of dedicated professionals provides crisis stabilization and medication management to patients who have disorders such as depression, anxiety, mood disorders, paranoia and schizophrenia. We provide a secure haven for patients who are having suicidal or homicidal thoughts. We can help patients regain their sense of self-worth. At Cedar Ridge Behavioral Hospital at Bethany, we offer an adult inpatient program that specializes in depression, bipolar disorder, anxiety and other mood disorders. The adult inpatient mental health treatment is for individuals over the age of 18 with severe mental illness that is impacting their ability to function.

Qualifications
Education:
- Bachelor’s degree in related field preferred
- High school diploma or equivalent required
Experience:
- Minimum one (1) year of case management experience required, preferably in the mental health field.
- Thorough understanding of the inpatient workflow process and step-down process as it pertains to case management and discharge planning preferred
Additional Requirements:
- Case Management Certification preferred
- Must be computer proficient and have strong communications and analytical skills
