Brentwood Behavioral Healthcare is seeking a dynamic and talented Community Support Specialist Full-Time.
The Community Support Specialist is responsible for coordinating and providing care that is safe, timely, effective, efficient, equitable, and youth and family centered. Services are primarily community-based, and participating individuals have mental health treatment needs and are in need of support additional to outpatient therapy services. The Community Support Specialist handles new referrals, creates service plans, reviews progress and makes determinations about closure and transitions to lower levels of care. The Community Support Specialist coordinates the flow of clinical information across agencies and providers while protecting the confidentiality of individuals served. The Community Support Specialist builds partnerships with enrolled individuals with the goal of helping them achieve wellness and autonomy.

Qualifications
Bachelor’s degree in social work, psychology, or a related human service field required, with at least two years of full-time experience providing direct mental health counseling and/or case management services preferred. Eligible for certification through MS DMH as a Community Support Specialist. Demonstrated skills in case management, crisis intervention, teamwork and collaborative plan development preferred
