Job Requirements:
- Current RN license in the State of Florida.
- Case Management certification.
- BS required. A. or M.S. strongly preferred
- Five (5) years Case Management Director Experience in an Acute Care setting required.

Role and Responsibilities
Assumes responsibility and accountability for the fiscal, operational and personnel management of the Case Management Department, including RN Case Managers & licensed social workers. Assures the achievement of Hospital inpatient and observation patient length of stay goals. Assures the placement of patients into the appropriate patient status. Collaborates with the CMO and Medical Staff on all Case Management/Physician issues. Coordinates safe discharge processes for all patients to the appropriate next step in the continuum of care. Develops plans to effectively manage patients post discharge to prevent readmissions. Hospital lead for the Medical Staff’s Utilization Management Committee. Assures the appropriate patients are referred to EHR for second level patient status reviews. Co-chairs the Hospital’s Denials Committee with the Patient Access Director, interceding to prevent patient denials and post discharge governmental and insurance company take backs.
