Via Linda Behavioral Hospital, an innovative behavioral health hospital offering a full continuum of outpatient and outpatient services to the people of Scottsdale and the greater Phoenix, Arizona metro area. Our 120-bed facility will offer specialized mental health services and substance use treatment for teens, adults and older adults experiencing issues such as depression, anxiety, personality disorders and co-occurring addictions.

Under the direction and supervision of the Director of Clinical Services, the Lead Discharge Planner provides critical support and care coordination the patients of Via Linda Behavioral Hospital. The Discharge Planner is responsible for assisting and coordinating with the Clinical Services team to assure that patients have excellent and safe discharge planning, as well as providing recommendations for post-discharge support. In addition, the Lead Discharge Planner assumes the responsibility of assisting to supervise the discharge planning department and its operations.


Qualifications

Skills / Requirements

  • A minimum of an Associate's Degree in behavioral health or equivalent behavioral health experience.
  • A minimum of 1 yeas of full-time behavioral health experience in a psychiatric health care facility.
  • Must have or be able to obtain a Level 1 Fingerprint Clearance Card.  
  • Special/ Other Skills:  Excellent written and oral communication skills, time management, crisis intervention, and group psychoeducation skills required.  Keep current with training and education in behavioral health.
