UltaCare Medical has an opening for an advanced level practitioner to provide primary care services onsite at a skilled nursing and long-term care senior living facility in Hillsboro, Illinois.
This role is exclusive to this one facility. In this role, you are the lead daily onsite clinician caring for the resident population and will also have the responsibility of coordinating care with their PCP/attending physician and/or the facilities Medical Director.
This role requires a compassionate, responsive, proficient healthcare professional that put's patients first. This is an institutional in-patient setting and this individual must be comfortable determining the medical necessity of residents as well as manage your time as you are one that determines the schedule of who to see and when. While you will work in a collaborative fashion with the sites Medical Director, the Medical Director is not on-site the majority of the time and so this individual will have to make many decisions independently. The role will report to the company's Chief Medical Officer who is also always available for guidance and support.

Essential Duties and Responsibilities:
- Provide medical evaluation, treatment, and consultation to facility residents.
- Perform chronic disease visits for residents on a scheduled frequency.
- Respond to, assess, and treat acute medical events as needed.
- Work in a collaborative fashion with all facility staff and the facilities Medical Director.
- Provide medical orders for prescription refills, administration of medications, ancillary testing, and/or referrals.
- Communicate to resident’s family members as needed.
- Fully document each patient interaction via dictation into a patient visit record within 24 hours of the visit.
- Reviews ancillary test results and coordinates notification to the patient.
- Provides patient education as needed.
Required qualifications:
- Are you a Board Certified Nurse Practitioner in Illinois?
Qualifications and Experience:
- Active Advanced Level Practitioner license. We are open to both NP's and PA's.
- Preferred experience working in an institutional inpatient setting.
- Preferred experience working in a skilled nursing and/or long-term care facility.
- Preferred experience working with geriatric patients.
- Individual needs to be comfortable and proficient at taking initiative with identifying patients to see, setting your own schedule, and forging and maintaining productive relationships with facility staff.
- Able to work independently, make own decisions.
Salary
- $105,000 - $150,000 annually.
