Social Work- EPD Wavier Case Manager

Our dedicated team at UHHS, provides high-quality in-home case management services to seniors and persons with disabilities; by partnering with caregivers, healthcare providers, and or social service agencies. We collaboratively work to implement a compassionate and professional care plan that is uniquely tailored for each client.

We are seeking to find a Case Manager (CM) to enhance the quality of life for our clients. The CM will interface with various internal and external stakeholders to support client needs through personalized care, compassionate communication and professional services. CM duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s health and human service needs. This can look like, the facilitation of treatment services, access and maintain Medicaid waiver and non Medicaid services, as well as support the access and retention of community supports.

Requirements and qualifications

• All staff are required to be vaccinated against COVID-19.
• Candidates must have current and appropriate licensure as a Nurse or Social Worker.
• Candidates must have at least one (1) year of experience working with seniors or individuals with disabilities.

Responsibilities

• Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered.
• Accurately manage case assignments through the continuous monitoring and support of the client and caregiver with monthly in-home visits, additional visits as needed and telephone calls.
• Assist clients and caregivers in making informed and educated decisions by acting as their advocate regarding their treatment options, waivers, support services and community resources.
• Provide continuous monitoring and support for client and family with monthly in-home visits, additional visits as needed and telephone calls.
• Respond to client or caregiver concerns within 24 hours of notification.
• Perform annual renewal of client Medicaid and recertification of waiver services in a timely manner.
• Accurately record all case management data and client information in casenet.
• Adhere to professional standards as outlined by organizational and ethical protocols, rules and regulations.
• Collectively work caregivers and healthcare providers to assist clients in achieving wellness and autonomy.

Job Type: Contract

Pay: $3,000.00 - $6,000.00 per month
