General Summary
The Social Worker is a professional caregiver who assumes responsibility and accountability for the assessment of patient needs in response to referrals from physician practices with the objective of connecting the patient and family with resources to address non-medical needs. Triages patients and assesses eligibility for benefits and programs, provides contact and facilitates referrals. Acts as a liaison between community physician practices, the community, and payer resources for care management. Maintains knowledge of and familiarity with community-based resources.

What You Need to Be Successful:
Qualifications
Education: Graduate of an accredited college with a degree in social work and current licensure in the State of Maryland.
Master’s degree with LGSW or LCSW certification required.
Experience: Minimum of 3-5 years of relevant clinical experience preferred. Ongoing education, certification, and self-development to remain current with industry standards and business objectives related to hospital care management a must.
