What are the responsibilities and job description for the Case Manager position at UMFS?
What we do:
- Individual, group, and family therapy
- Medication management and weekly psychiatry sessions
- Psycho-education and independent living groups; recreational and pet therapy
- Social-skill development through experiential and project-based learning
- Individualized academic instruction on SOL curriculum and IEP goals (led by onsite Fairfax County school teachers)
- Trauma histories
- Mood and anxiety disorders
- Emotional, social, and behavioral challenges

What we are looking for:
A Clinical Case Manager to provide individual, family, and group therapy and case management services to youth and their families who are experiencing a mental or emotional challenge which has impacted their ability to safely remain in a community setting. Case management includes compliance with funding documentation as it relates to medicaid, FAPT, and Fairfax county CSA requirements as well as linking to discharge services and administrating risk assessments for youth in the program. Serve as liaison for clients and their families with internal and external resource agencies.
What you will need:
(LCSW or LCP eligible) Master’s degree in social work, psychology, counseling or other human service degree required, and 2 years of experience providing clinical and case management services to children and families is preferred. Residential experience preferred.
