POSITION DESCRIPTION
General Summary
UMIT Health System is currently operating a clinic, pharmacy, and Lab in King William County and is expanding into home healthcare. UMIT Home Health is providing in-home skilled services (Nursing, PT, and OT) and in-home personal care services. Providing high quality healthcare to our patients is important to us, and we are looking for individuals that strive to deliver high quality healthcare and positive outcome for patients.
Position
Register Nurse (RN) – Home Health
Completes evaluation and documents patient need for in home patient care services. Provides coordinated skilled nursing care to patients of all age groups, in the home. Demonstrates accountability and responsibility in ensuring patient goals are achieved while maintaining high quality patient care. Perform all services in accordance with physician’s orders.
Primary Duties and Responsibilities:
- Conduct an initial and ongoing comprehensive assessment of the client’s needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate time points.
- Conduct a physical examination of the client, including vital signs, physical assessment, mental status, appetite and type of diet, etc.
- Evaluate the client’s condition and home situation to determine if the services of a personal care nurse (CAN) will be required and the frequency of this service.
- Nursing services, treatments and preventative procedures requiring substantial specialized skill and ordered by the physician.
- The initiation of preventative and rehabilitative nursing procedures as appropriate for the client’s care and safety.
- Observe signs and symptoms and report to the physician reactions to treatments, including drugs, as well as changes in the client’s physical or emotional condition.
- Deliver compassionate support, attention, and assistance to patients and families.
- Assist and train personal care nurses with assigned patients, as needed
- Travel required within the service area (mileage reimbursement provided).
- Ensure compliance with all health care regulations, including HIPAA and OSHA.
Knowledge, Skills and Ability Qualifications:
- Graduation from an Accredited RN Program.
- Valid Licensed, Registered Nurse (required).
- Five years of experience in a medical setting (preferred).
- Ability to think and work independently and with direction.
- Ability to communicate with staff members in a fast-paced and stressful environment.
- Maintain a basic understanding of patient care, medical terminology, coding procedures, reference tools, and appropriate clinical pharmacology for medical assistant practice scope.
- High School graduate or equivalent.
- Strong computer and documentation skills
