Unicare Home Care is looking to hire a full-time CDPAP Coordinator for our Brooklyn office. As a CDPAP Coordinator, you will play an integral part in the agency.

CDPAP COORDINATOR POSITION SUMMARY:

This CDPAP Coordinator position calls for assisting with day to day operations of the coordination department.

Such duties are as follows :

Educate and guide potential Personal Assistant's on CDPAP
Coordinate Personal Assistant on boarding
Intake on CDPAP consumers
Manage weekly schedules on CDPAP consumers to maintain clock in /out based on EVV
CDPAP Home visits for new intake Review all incoming referrals and conduct initial screening to ensure they meet the defined admission criteria
Conduct informational and screening interviews with patients and their families as part of the admission process
Enter Client- specific data into the computer system

CDPAP COORDINATOR EXPERIENCE:

CDPAP Coordination: 1 year preferred

Home Care: 1 year required
