Case Manager

Position: Case Manager/Social Worker

We have been accessing services for Miami-Dade County for over 26 years and continue to show excellence. Our goal is to establish a well rounded base of Case Manager/Waiver Support Coordinators, who will be the face of Unike Support Services INC.

Job Duties:

1. Meet with consumers, families, and providers on a monthly basis.

2. Maintain case notes every month for each consumer.

3. Gather and inquire information about the consumer. (Health, safety, well-being, and education etc.)

4. Creating a plan of service (Yearly)

5. Responsible to obtain and inform our consumers of community resources and events that would allow the consumer better integration in the community.

6. Office duties

7. Traveling in Miami-Dade.

Job Requirements:

Each Candidate MUST have the following criteria:

1. Bachelors Degree (REQUIRED)***

2. Two years experience as a case manager/social worker OR related field (REQUIRED)***

3. Typing Speed: 50 WPM

4. Native or professional knowledge of the English language. (REQUIRED)***

5. Native or professional knowledge of the Spanish language. (REQUIRED)***

Benefits Include:

1. Full Time Position (Social Worker/ Case Manager).

2. Gas reimbursement.

3. Starting salary ranges: $40,000-$42,000 (Depending on Experience)

4. Other benefits: cell phone allowance, vacations, paid legal holidays, etc.
