POSITION DESCRIPTION
JOB TITLE: Home Manager
Department: Community Living Services
SUPERVISOR: Regional Director
FLSA STATUS: Non exempt
JOB SUMMARY: Responsible for the day-to-day operations of one residential home.
EDUCATION OR TRAINING REQUIREMENT: High School diploma or equivalent. Associates degree preferred.
EXPERIENCE REQUIREMENT: Minimum 2 years’ experience working with individuals with disabilities and/or related field; 1 year of supervisory experience preferred; Equivalent field experience may be substituted for supervisory experience with Chief Program Officer.
SPECIAL LICENSES, REGISTRATION OR CERTIFICATION: GA Driver’s License and vehicle insurance; must have ability to utilize Microsoft Office Suite (Word, Excel, Power Point, Outlook, etc.)
SUPERVISORY RESPONSIBILITY: Supervises in-home, direct support professionals for assigned residences
ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:
I. Training/Program Responsibilities:
A. Ensures person-centered activities based on interest of the individuals and choice are coordinated and implemented. These activities should include activities focused on goals.
B. Promptly reports any emergency/crisis situation to supervisor and follows up by completing appropriate incident report within twenty-four (24) hours.
C. Coordinates transportation and supervision for individuals as dictated by individual program.
D. Ensures that all information regarding daily activities, problems and needs of individuals are recorded properly in Therap.
E. Provides training in changing appropriate apparel on a daily basis to ensure comfort and well being for individuals.
F. Ensures that proper maintenance is performed on the van when necessary.
G. Attends annual ISP meeting for each individual.
H. Is responsible for staffing the homes, including overseeing AOD reporting, relief staff, and leave scheduling.
I. Acts as liaison person between staff and the Consumer Support team
J. Participate in on call rotation
II. Relating to Health and Safety:
A. Inspects home to ensure that home meets all safety requirements, checking for clutter and dangerous objects.
B. Corrects deficiencies or submit written report to supervisor outlining deficiencies by end of shift.
C. Coordinates evacuation drills, both fire and severe weather; records results of each drill.
D. Follow program plan for meeting all potential emergencies and disasters, such as medical, fire, severe weather and missing persons. Discusses with support team specific role relating to each emergency.
E. Notifies supervisor of all medical emergencies as outlined by policies and procedures.
F. Checks to ensure that supplies are clearly labeled.
G. Reports any problem with equipment used by individuals to proper individual or department
H. Ensures cleanliness of home
III. Housekeeping Responsibilities:
A. Ensures that laundry is washed, ironed and properly put away daily.
B. Ensures menus are nutritionally sound and appropriate for all individuals in the homes; ensures menus are posted for at least 30 days in accordance with regulations and previous three months menus are filed in the home
C. Orders groceries for the home in accordance with approved menus.
D. Purchases required SMS and SME; coordinates the funding for such purchases.
IV. Administrative Responsibilities:
A. Ensures compliance in the area of responsibility during assigned hours with local, state and federal rules and regulations applying to Community Living Arrangements licensure, UCP, & Community Living Services.
B. Complies with the organization policies and procedures governing the use of behavior management programs for controlling maladaptive or problem behavior.
C. Complies with policies and procedures to ensure that confidentiality requirements are upheld.
D. Supports and protects the fundamental human, civil, constitutional and statutory rights of clients and families as defined by agency.
E. Demonstrates competence in handling medical emergencies in accordance with UCP policies and procedures.
F. Maintains accurate and complete records of all individual’s personal spending monies in accordance with rules and regulations; completes monthly reconciliation and submits report to Regional Director of Community Living Services for approval and final submission
G. Maintains accurate and complete records of house spending (including credit card purchases and petty cash purchases) in accordance with rules and regulations; completes monthly reconciliation and submits reports to Regional Director for Community Living Services for approval and final submission
H. Reports to work promptly and on time and does not leave work until assigned shift ends or has approval from supervisor. Observes policies/procedures governing requests for annual, sick or other leave time.
I. Observes all other personnel policies and works closely with supervisor to ensure compliance.
J. Abides by code of ethics and serves as a good role model for clients.
k. Runs weekly Relias reports and provide necessary follow up with staff regarding training and personnel requirements.
V. Record Keeping Responsibilities:
A. Maintains records in the home to meet local, state and federal guidelines
B. Reviews Therap Daily for completion of shift notes and Goal tracking- ensure there are no GAPs in notes. Corrections should be made within 24 hour.
C. Maintains tracking records in accordance with Individual Service Plan to comply with program requirements and expectations of supervisor (bowel tracking, intake/elimination, seizure, weights, etc).
D. Records contacts with individual’s family or guardian.
E. Maintains current log for approved family visitations and/or outing with friends.
F. Maintains medication records in accordance with policy and completes weekly medication count
G. Maintains records of evacuation drills
H. Maintains appropriate documentation relating to health, safety and infection control.
I. Maintains records of van log data sheets.
VI. Physical Health Service Responsibilities:
A. Receives training on supervising the self-administration of medications and follows all policies and procedures.
B. Assumes responsibility for ensuring that all medications are kept under lock and key.

C. Obtains annual TB test and ensures that results are signed by appropriate medical staff and on file in Center Personnel record.
D. Considers the need and advisability of obtaining Hepatitis B Vaccine in accordance with UCP recommendation.
E. Provides required documents for personnel file as requested, to include Georgia Driver’s License, First Aid/CPR certification, physical, etc.
VI. Other:
A. Adheres to all agency policies and procedures and Community Living Arrangement.
B. Develop and maintain positive and productive relationships with individuals, families, co-workers, and community members.
C. Maintain objectivity in position in order to set appropriate limits while working with consumers.
D. Support individuals in their personal growth and development, respecting cultural, ethnic, spiritual, and individual differences
E. Adhere to all agency policies and procedures
F. Other duties as assigned
