SUPERVISOR: Coordinator of Community Living
FLSA STATUS: Full time, Non-Exempt
JOB SUMMARY: Responsible for the day to day operations of a single residential location ensuring the provision of residential habilitation services.
EDUCATION OR TRAINING REQUIREMENT: High School diploma or equivalent. Associates degree preferred.
EXPERIENCE REQUIREMENT: Minimum 2 years experience working with individuals with disabilities and/or related field; 1 year of supervisory experience preferred; Equivalent field experience may be substituted for supervisory experience with Executive Director’s approval.
SPECIAL LICENSES, REGISTRATION OR CERTIFICATION: Valid SC Drivers License with safe driving record and valid vehicle insurance; must have ability to utilize Microsoft Office Suite (Word, Excel, Power Point, Outlook, etc.)
SUPERVISORY RESPONSIBILITY: Supervises Community Living Coach staff
ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:
- Staff Supervision and Development
- Provide direct supervision of staff to ensure individuals served receive residential habilitation as required.
- Oversee the 24 hour staff support at all assigned residential sites. Schedule and conduct staff meetings and training to ensure on-going skill development of staff.
- Directly train staff on all changes regarding residential habilitation, behavior supports, health and safety supports and policy changes.
- Responsible for the hiring, evaluating and coaching of staff.
- Provides on-call support to staff regarding needs/issues of the individuals served, staffing issues, and other areas of concern as needed.
- Training/Program Responsibilities
- Ensure person-centered activities are based on the interests of the individuals and ensure coordination and implementation of the events.
- Promptly report any emergency/crisis situation to supervisor and complete appropriate incident report form.

Coordinate transportation and supervision for individuals as outlined by individuals plan(s)
- Ensure that all information regarding daily activities, problems and needs of individuals are recorded properly.
- Ensure that proper maintenance of agency vehicles is maintained.
- Attends required meetings for each individual as scheduled
- Responsible for scheduling of staff in the home to ensure appropriate coverage at all times.
- Serve as a member of the Individual Support team
- Participate in on call rotation
- Relating to Health and Safety
- Ensures that the home is safe, sanitary, and free from obvious hazards at all times.
- Corrects any health and safety hazards immediately and/or report by phone or appropriate systems.
- Ensures housekeeping responsibilities to include but not limited to, kitchens, bathrooms, windows, bedrooms, and floors are successfully completed by staff
- Ensure successful completion of evacuation drills. Ensures the result of each drill is recorded by staff.
- Ensures all staff adheres to agency protocol for all potential emergencies and disasters. Maintains documentation as required.
- Ensures all staff adheres to policies and procedures for all medical emergencies.
- Ensures that food is properly stored, labeled as required, and protected from contamination and spoilage.
- Ensures the home is in compliance with all agency, state, and federal regulations pertaining to licensing and or other requirements.
- Ensures the proper delivery and documentation related to medications by daily review of the individual Medical Administration Record(s) for each home.
- Ensures menus are nutritionally sound and appropriate for all individuals in the homes. Ensures weekly menus are posted in accordance with regulation and previous menus are filed in the home
- Ensure the purchase of groceries for the home on a weekly basis, in accordance with approved menus.
- Ensures he home is in compliance with all agency, state, and federal regulations pertaining to licensing and or other requirements.
- Administrative Responsibilities
- Ensures compliance with agency, state and federal rules and regulations governing residential habilitation.
- Complies with the organization policies and procedures governing the use of behavior management programs for controlling maladaptive or problem behavior.
- Complies with policies and procedures to ensure that confidentiality requirements are upheld.
- Supports and protects the fundamental human, civil, constitutional and statutory rights of individuals and families as defined by human rights counsel.
- Ensures staff reports to work promptly and on time and does not leave work until assigned shift ends or has approval from supervisor. Observes policies/procedures governing requests for annual, sick or other leave time.
- Utilize time management skills to complete assignments within scheduled shifts
- Abides by all other personnel policies and works closely with staff to ensure compliance.
- Maintains records in the home to meet agency, state and federal guidelines
- Ensures individual related entries in daily logs that are legible, dated, authenticated by signature and positions in ink and without use of symbols or abbreviations.
- Ensures electronic tracking records are maintained in accordance with the individual’s plan(s) to comply with program requirements.
- Ensures contacts with client’s family or guardian are recorded. Communicates with Individual support team according to individuals residential support plan as needed.
- Ensures staff maintains accurate and current log for approved family visitations and/or outings with friends.
- Ensures staff maintains accurate medication records in accordance with policy
- Ensures staff maintains appropriate documentation relating to health, safety and infection control.
- Ensures staff maintain accurate and complete records of all individual’s personal spending monies in accordance with rules and regulations; completes monthly reconciliation and submits report to Regional Director of Community Living Services for approval and final submission
- Ensures staff maintains accurate and complete records of house spending (including credit card purchases and petty cash purchases) in accordance with rules and regulations; completes monthly reconciliation and submits reports to Coordinator of Community Living Services for approval and final submission
- Report to work promptly and on time and does not leave work until assigned shift ends or has approval from supervisor. Observes policies/procedures governing requests for annual, sick or other leave time.
- Observe personnel policies and works closely with supervisor to ensure compliance. Abide by code of ethics and serves as a good role model for individuals and staff.

Manages budget effectively including staying within allocated staffing hours, allocated
grocery budget, maintaining household supply budget, and utility expenditures.
- Other Required Duties:
- Maintains required trainings and certifications as required to remain in compliance with agency, state and federal guidelines.
- Adheres to all agency policies and procedures and SCDDSN standards.
- Must be able to lift at least 50lbs, bend, squat, stand, walk frequently throughout the shift.
- Operate and maintain a high degree of professionalism and demonstrate leadership ability.
- Perform all duties required of the Community Living Coach as needed
