Requirements
The School Based Clinician provides direct service to staff, students, teachers and parents of children in the Bennington, Arlington, or Manchester school districts, allowing on-site consultation and service in a public-school setting.
MAJOR RESPONSIBILITIES:
DIRECT CLINICAL SERVICES: Provides individual, family or group counseling services to parents and children in the community or school, as needed. The School Based Clinician will identify the mental health and behavioral needs of students; develop and implement treatment plans; conduct clinical interviews, assessments, observations, and complete clinical reports; review referrals of students who need intensive mental health services; provide support in the development of behavior support plans; conduct crisis and/or risk assessment and response, as needed.
CASE MANAGEMENT / SERVICE COORDINATION: Makes home visits to support families, as needed. Facilitates communication between agencies on behalf of the student. Attends team meetings to plan and monitor services. Provides case management, collateral contact and social support, as needed.
CONSULTATION/EDUCATION SERVICES Provides consultation services to teachers and school personnel, as needed. Consultation can be educational or evaluative.
CLINICAL RECORD-KEEPING: Initiates and maintains, in a timely and thorough manner, the wide variety of record keeping and administrative documentation required to account for services provided to consumers – including, but not limited to, consumer specific assessments, case services, progress notes, time sheets, schedules, billing reports to third party payers and assigned periodic reports. Follows all UCS record-keeping procedures and performs other duties as necessary.
ADMINISTRATIVE: Attends all UCS and community meetings necessary to promote coordination and communication. Follows all UCS record-keeping procedures and performs other duties as necessary. The school-based clinician is required to meet or exceed the billing case rate as defined under the Success Beyond Six contract.
