Hospice RN/LVN Field Nurse is primarily responsible for a comprehensive nursing assessment and follow up visits to hospice patients. This position enables patient to receive the best quality of home health care through appropriate nursing intervention in accordance with plan or care.

HOSPICE RN FIELD NURSE
Under the supervision of Director of Patient Care Services, the RN is primarily responsible for planning, organizing and directing home health care, provides field services, case management, administration, community relations, and other functions and responsibilities.
Responsibilities and Duties:
- Committed in bringing best quality of patient care services during home or facility visit.
- Ability to administer effective care and treatment for patient and family in home or facility setting
- Provide adequate education on appropriate usage of medications, DME and supplies
- Ability to perform nursing intervention as ordered by Attending Physician and Home health Agency
- Communicates with physician, caregivers, family, home health team members for any changes in patient condition and commences plan of care as needed
- Be familiar with Government guidelines
- Completes all reports in a timely manner
- Maintains confidentiality in accordance to HIPAA compliance
- Performs job in compliance with Agency policies, procedures and applicable laws
Requirements for Position:
- Graduate of Bachelor's Degree in Nursing
- Valid California RN/LVN License
- At least 1 year experience in either of the following:
*Home Health
*Hospital
*Subacute
- Valid California Driver's License
- Reliable Transportation
- Proof of auto insurance
Bilingual: Spanish and Chinese Speaking is a plus.
