United Hospice, Inc. is a community-based, not-for-profit hospice that provides care and support to the communities of Rockland and Orange Counties, New York. We are currently seeking to fill a full-time LPN position for the Joe Raso Hospice Residence in New City, Rockland County.
This is a full-time, benefits eligible position. United Hospice offers medical, dental and vision insurance. We have a 401(k) retirement plan with profit sharing, a generous paid time off program, and additional perks.
The LPN will provide patient care under the supervision of an RN. EMR documentation experience required. Applicant must demonstrate critical thinking skills, basic knowledge of nursing theory and excellent teamwork skills. Must be available to work weekends and holidays as needed. NYS LPN license and minimum of two years clinical experience is a must.
Responsibilities:
- Provides direct patient care as defined in NYS Nurse Practice Act, under the supervision of a registered nurse
- Actively participates in coordination of all aspects of patient’s care, in accordance with current professional standards and practice
- Ability to provide direct care according to the established patient plan of care
- Complete, accurate and timely documentation of services rendered as outlined on the patient plan of care
- Contributes to patient and family counseling and education
- Provides appropriate pain/symptom management according to the patient’s plan of care
- Keeps registered nurse apprised of patient’s condition
- Adheres to all policies and procedures established by the agency
- Uses equipment and supplies appropriately
- Maintains patient confidentiality at all times
- Participates in ongoing interdisciplinary comprehensive assessment
- Accepts other responsibilities as assigned
- Maintains Point of Service documentation at a minimum rate of 80%
- Participates in QAPI program and hospice sponsored in-service training
- Incorporates organization’s commitment to customer service and continuous quality improvement into daily work habits. This includes:
- Promotes positive co-worker and team work relationships
- Supports the goals and mission as set forth by the Board of Directors and administration of the agency
Qualifications:
- Graduate of an accredited practical nursing program
- Possession of a valid and current NYS Practical Nurse’s license
- Minimum of 2 years’ experience in nursing is required
- At least two years recent Med-Surg nursing experience preferred
- Hospice experience is not required but would be a plus
- Complies with accepted professional standards and practice
- Commitment to the Hospice Philosophy of Care
- Ability to relate to a diverse population and workforce in a personable and effective manner
