Serves as an expert resource for all aspects of nursing.
Listens to report on all patients on unit.
Reviews all information.
Conducts patient rounds with each nurse and evaluates goals and plans of care for all patients.
Monitors each patient’s progress with the plan of care and adjustments as needed.
Collaborates with interdisciplinary team members to identify potential barriers and variances to the plan of care.
Acts as a liaison between patients, caregivers and community resources and all departments.
Promotes development of evidenced based clinical practice.
Utilizes disease specific protocols to meet patient goals.
Promotes patient safety.
Demonstrates creativity and innovation in approach to complex problem solving.
Good communication & teamwork skills.
Knowledgeable of all policies and procedures.
Working knowledge of all normally used equipment
One (1) years nursing experience
Graduate of an accredited School of Nursing. BS degree in nursing required
Current D.C. licensure as a Registered Professional Nurse
Have and maintain current Basic Life Support (BLS)
Attends in-service and education programs; attends continuing education required for maintenance of professional certification or licensure
Delivers and maintains optimum patient care during Hem dialysis through safe technical skills and thorough understanding of the physical principles of the Dialectic procedure
Essential Duties and Responsibilities:
Assembles supplies
Sets up machine
Primes dialyzer
Reviews low dose heparin and standard heparin policies
Prepares heparin and adjusts heparin dosages
Prepares dialysate, test conductivity and checks temperature
Calculates TMP for negative pressure mode
Utilizes ultrafiltration mode UF or négative pressure mode on Cordis Dow 480
Drains UF collection bag utilizing appropriate precautions with effluent
Performs ACT procedure
Delivers and maintains optimum patient care during Hem dialysis through safe technical skills and thorough understanding of the physical principles of the Dialectic procedure.
Obtains necessary blood work
Checks and sets alarms
Monitors patient as necessary
Adjust negative pressures and blood flow rates
Performs dressings
Maintains aseptic technique and unit isolation precautions
Administers medication according to IPD procedure
Documents all information
Assists with A-V shunt declotting, vascular catheters insertion
Change dialyser during dialysis
Change blood lines during dialysis
Performs take off procedure
Develops patient care plans
Call reports to floor nurse concerning dialysis procedure
Applies cardiac monitor and observes for arrhythmias
Locates code button, emergency cart, and cardiac monitor
Checks water treatment system and states unsafe gauge levels
Performs DPT tests for chloramines correctly
Terminates dialysis procedure by sterile technique and accurately assesses patient post treatment status
