The MDS Nurse Coordinator manages all documentation and assessment related to MDS. Such management must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be required by the Director of Nursing Services.
What you’ll do:
- Coordinate and/or complete the assessment and evaluation of newly admitted residents.
- Coordinate, complete, and review Minimum Data Sets (MDS) for all residents.
- Translate MDS into an accurate care plan for each resident.
- Transmit completed MDS to State of Michigan in accordance with applicable rules.
- Attend care conferences with appropriate interdisciplinary team members as needed.
- Perform assigned rounds on nursing units to ensure regulatory compliance and resident satisfaction.
- Educate and train nursing staff and other interdisciplinary team members regarding MDS process.
- Assist other departments with MDS completion as required.
- Ensure all MDS updates and changes are implemented, and appropriate staff is trained.
- Serve in “on-call weekend rotation" and "one scheduled day per week on call rotation" capacity as required, and as needed by the nursing department or assigned by DON.
- Perform other duties as assigned by the Director of Nursing.
- Perform nurse staff replacement as needed assigned by DON.
- Assist with answering call lights as needed while on nursing units.
- Attend various committee meetings of the facility (i.e., Quality Assessment, etc.) as required.
- Provide information to the Quality Assurance and Assessment Committee as requested.
- Manage and direct/lead weekly Medicare /PPS meeting.
- Maintain the confidentiality of all resident care information.
- Assure that residents rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to register complaints, are well established and maintained at all times.
- Must be able to move intermittently throughout the workday. Sit, stand, bend, lift and move intermittently during working hours.
- Must be involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.
- Must function independently, and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.
- Must be in good general health and demonstrate emotional stability.
- Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility.
- Must be able to assist in the evacuation of residents during emergency situations.
- Must communicate with medical staff, nursing personnel, and other department supervisors.
- Is involved in community/civic health matters/projects as appropriate.
- Attends and participates in continuing educational programs.
What it takes:
- A valid Michigan nursing license from an approved school of nursing (RN preferred).
- As a minimum, one (1) year of experience as a staff nurse in a hospital, long-term care facility, or other related health care facility.
- Training in rehabilitative and restorative nursing practices preferred, but not required.
- The ability to make independent decisions when circumstances warrant such action.
- The ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
- Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
- The ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care.
- Must be able to relate information concerning a resident's condition.
