This role actively engages with patients both in the home and by telephone. The incumbent will use compassion and empathy to earn patients’ and caregivers’ trust, persistence to support and advocate for them, and excellent communication skills to build impactful relationships.SUPERVISORY Reports directly to the Supervisor, Clinical Nurse Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 8am - 5pm. It may be necessary, given the business need, to work occasional overtime. Our office is located at 212 W Washington St Chicago, IL. Employees will be required to work somedays onsite and somedays from home*All Telecommuters will be required to adhere to UnitedHealth Group’s Telecommuter Policy.Primary Responsibilities:Conduct home visits and/or phone or video calls to patients to collect clinical and psychosocial information, and carry out designated tasks, including but not limited to:Resource referral and connectionQuality measure documentation, support, and follow upTelemedicine facilitation and set upCollect information regarding patient conditions using standardized checklists and templates.Partner with Landmark interdisciplinary team (providers, nurses, social workers, etc.) to drive improvement in patient outcomes and satisfactionActively participate in weekly interdisciplinary team meetingsYou’ll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Requirements:High School Diploma / GED (or higher)3-5 years of experience in any of the following job categories or categories similar to: Social Worker; Home Health Aide; Certified Nursing Assistant; Certified Medical Assistant; Medication Aide; Health Coach; Health Navigator; Healthcare Technician Ability to conduct independent home visitsExperience with Microsoft Office products (Word, Outlook, and Excel)Access to reliable transportation required; if you are driving a vehicle, you must comply with all the terms of the Landmark Motor Vehicle Safety policy.
