Responsibilities
Client Service
- Assist and provide treatment goal development per the treatment plan and according to the needs of clients and assist in the develop goals that will allow them to achieve the greatest level of independence in their community.
- Share information with program supervisors regarding significant events and / or client responses that vary from their baseline, as it relates to their treatment plan.
- Advocate for clients to help meet their needs, desires and interests.
- Advocate for clients during community integration activities, appointments or anytime in the community.
- Encourage client independence by assisting with a wide variety of daily living skills.
- Provide medication administration, following the established rules of medication administration.
- Transport clients to community outings, appointments and as part of their treatment plan.
- Arrive to work on time and work as scheduled.
Documentation
- Complete all documentation timely, accurately and within established guidelines.
- Ensure all service documentation meets all established billing requirements.
- Complete incident reports timely, accurately and within established guidelines.
- Record any contacts with parents, guardians, care coordinators or any other stakeholders.
- Complete all medication administration documentation thoroughly.
Qualifications
Education : High School diploma or GED Associate’s Degree in Social Services or related field.
Experience : Preferred experience working with the population served.
License(s) / Certification(s) : Maintain a valid driver’s license when driving any vehicle for work-related reasons. Adult CPR, First Aid and Mandatory Dependent Adult Abuse certification.
Successful completion of a Medication Management course that is approved by the Iowa Department of Inspections and Appeals and / or HCBS, within six-months of hire.
Knowledge / Skills / Abilities : Ability to speak, read and write fluent English. Ability to read and comprehend common terminology of the mental health field.
Ability to solve practical problems and deal with a variety of variables. Must possess good interpersonal skills and ability to work under pressure.
Ability to remain emotionally neutral when intervening with clients. Ability to establish and maintain boundaries with clients.
Must enjoy working with people.
Other : Support and abide by departmental and agency policies and procedures. Perform job in safe manner, insuring safety for self, team members and residents.
Perform other duties as assigned.
